Shift Type: Per Diem

Onsite Care seeks a New Jersey licensed Registered Nurse for in-home visits to our clients throughout Essex County, Hudson County, and Union County.

Register Nurses prepare thorough and accurate Initial Assessments and Reassessments and develop personalized care plans. This per diem position is ideal for a registered nurse who desires significant work but with flexible hours.

Home Care Register Nurse responsibilities include:

• Creating in-home initial client assessments and care plans based on clients' specific care needs
• Conducting follow-up in-home assessments at specified intervals
• Conducting periodic telephone assessments
• Revising care plans as necessary
• Case management
• Supervising Certified Home Health Aides (CHHAs)
• Performing Orientations/Skills Assessments of CHHAs
• Excellent observational and problem-solving skills.
• The ability to follow instructions but also to act independently if necessary.
• Excellent report writing skills.
• Active listening skills and empathy.
• Physical and mental strength.
• A valid driver’s license and reliable transport may be required.

Qualifications include:

· Strong preference for prior Home Care RN experience

· Current New Jersey Registered Nurse license

· Enjoy meeting with and talking to older adults

· Previous RN experience with in-home care assessments

· Case Management experience

· Excellent written and verbal communication skills with strong interpersonal skills

· Must be self-motivated, organized, and able to work independently

· Willing and able to travel to clients' homes

· Must have a valid driver's license and a reliable car, and be willing to travel to clients' homes

· Basic computer skills required (data entry on a web-based program, email, scanning)

· Weekday flexibility

Pay: $40.00 - $50.00 per hour

Expected hours: 20 per week
