Open Door Family Medical Center provides health care and wellness programs to individuals and families in need throughout Westchester, Putnam and Ulster Counties. Open Door is seeking aCare Manager, Youth Treatment & Recoverylocated out of our Purchase, NY location. The Care Manager, Youth Treatment & Recovery Program provides ongoing care coordination service to patients enrolled in the Family Tree Program which is and initiative to enhance comprehensive treatment and recovery services for vulnerable adolescents and transitional aged youth with substance use disorders (SUD) and/or co-occurring mental health disorders (COD). Following the patient-centered medical home health care delivery model, the Care Manager participates in a patient-focused, team-based approach to provide direct and collaborative care coordination to patients enrolled in the program. As part of the medical care team, the services provided by the Care Manager aims to improve patient health outcomes and assist patients in taking control of their health care and becoming independent, and informed consumers of health care, substance use treatment and mental heath services. Primary responsibilities include assessing patients’ needs, risks and barriers (social determinants of health), developing of care plans and assist patients navigating medical, substance use treatment, mental health, and social/community services.

RESPONSIBILITIES

CARE MANAGEMENT

  • Outreach individuals and engage them in the Family Tree Program
  • Conduct assessments, in accordance with programmatic guidelines, to identify enrollees’ individualized needs related to psychosocial wellbeing, physical health and the social determinants of health; utilize standardized assessment tools; monitor needs on an ongoing basis
  • Based on the findings from the assessments, collaborate with Family Tree enrollees to develop a plan of care
  • Engage Family Tree enrollees in the goal setting process and utilize Motivational Interviewing and other techniques to assist in goal achievement
  • Coordinate and arrange for the provision of medical, behavioral health and other relevant services.
  • Accompany Family Tree enrollees to appointments as needed; model and coach on medical/behavioral visit navigation with the aim of empowering Family Tree enrollees to become independent, effective health care consumers
  • Conduct home visits as needed
  • Support Family Tree enrollees in adherence to treatment recommendations from medical and behavioral health providers
  • Provide psychoeducation using one-on-one teaching, written and printed materials as well as computer and video instruction approved by the primary care team to assist with Family Tree enrollees’ understanding of physical and behavioral health conditions and treatments
  • Identify available community-based resources and actively manages appropriate referrals, access, engagement, follow-up and coordination of services
  • Promote evidence-based wellness and prevention by providing health education on the following topics: substance use and addiction, mental health, chronic medical conditions (i.e. diabetes, asthma, hypertension, and HIV/AIDS) to improve adherence to treatment and plans of care
  • Link Family Tree enrollees to services/resources based on individual needs and preferences such as peer support programs, support groups, smoking cessation, and self-management programs
  • Discuss advance directives and health care proxies with enrollees and their families or caregivers
  • Retain Family Tree enrollees in care for a period of at least 6 months
  • Travel as required to community-based meetings, medical/behavioral health provider sites and meetings with Family Tree enrollees
  • Represent Open Door at community health fairs and community outreach activities
  • Complete all documentation and data entry activities as required for the Family Tree Program

COMMUNICATION

  • Informs supervisor promptly in any event that may affect quality and standards of patient or that impact the success of the Family Tree Program
  • Demonstrates the ability to function in a culturally sensitive manner within Open Door’s philosophy and mission
  • Actively promotes planned care concepts within Open Door

PLANNING

  • Attends staff and other organizational meetings, as designated, to ascertain Open Door’s short- and long-range plans, programs, and goals within areas of responsibility
  • Participates in staff development programs, educational activities, seminars, and training opportunities

COMPLIANCE MANAGEMENT

  • Adheres to organizational and federal requirements regarding data and confidentiality, electronic transmission protocols, and security and maintains patient confidentiality at all times per Open Door policies and procedures
  • Adheres to National Patient Safety standards as defined by the Joint Commission on Accreditation of Healthcare Organization and the Open Door Family Medical Centers
  • Prepares and maintains documentation in compliance with Joint Commission standards and applicable city, state, and federal laws
  • Meets annual mandatory training requirements

QUALIFICATIONS

EDUCATION

  • Master’s Degree preferred; candidates with a Bachelor’s Degree and solid work experience will be considered as well
  • Degree in Social Work or social sciences preferred; other degrees with relevant experience will be considered
  • Certification or coursework in addiction, substance use disorder treatment and management preferred (e.g. CASAC or CASAC-T/CASAC-P)

EXPERIENCE

  • At least three years of experience in substance use treatment, mental health, healthcare and/or social services settings
  • Health education experience is preferred
  • Experience with electronic medical records is preferred
  • Knowledge of/experience in case management and care coordination is preferred
  • Experience working with young people aged 12-25

  • Culturally competent/sensitive (racial, ethnic, linguistic, LGBTQ , physical ability, etc.) and committed to inclusive, multi-cultural programming
  • Comfortable providing direct patient care to patients age 12 to age 25 and with significant medical, mental health, and substance use needs
  • Proficient computer use, including Microsoft Office applications
  • Strong writing and reading skills
  • Able to understand and follow detailed instructions
  • Highly organized and self-directed, able to multi-task
  • Flexible and exhibits the ability to adapt as situations evolve
  • Comfortable working in a fast-paced, high accountability environment
  • Effective in problem solving and exhibits excellent judgment and the ability to prioritize

REQUIREMENTS

  • Must have valid NYS driving license and daily access to car
