Overview:
We are seeking a dedicated Case Manager to join our team. The ideal candidate will have a passion for helping individuals in need and possess a strong background in social work, crisis management, and motivational interviewing.
Duties:
- Provide case management services to clients in need of support
- Conduct assessments to determine clients' needs and develop individualized care plans
- Collaborate with clients to set goals and monitor progress towards achieving them
- Coordinate services with other agencies and providers to ensure comprehensive care
- Advocate for clients' rights and access to resources
- Maintain accurate and up-to-date case notes and documentation
Requirements:
- Bachelor's degree in Social Work or related field (Master's degree preferred)
- Previous experience in addiction counseling, child welfare, or behavioral health
- Knowledge of Medicare, Section 8, and workforce development programs
- Strong supervisory skills and the ability to work effectively in a team setting
