Open Sky Community Services is Hiring a Director - Community Health Partnership Near Worcester, MA


Description and Responsibilities:

The Central Community Health Partnership (CCHP) is a collaboration of five premier human service organizations in Central Massachusetts that has been selected to provide Behavioral Health Community Partner services for people with Behavioral Health Challenges. We help individuals with complex needs navigate access to services and assist in finding the right combination of services, as it can be an overwhelming task. The vision is integrated care that ensures that every member receives the right care at the right time making good health and wellness a reality, not a dream.

The Director of the Central Community Health Partnership will provide leadership and management of CCHPs Behavioral Health (CCHP/BH) Community Partners Program and CCHPs Long Term Services and Supports (CCHP/LTSS) Community Partner Program. The Director will supervise the centralized core team at CCHP and will work under the direction of agency executive leadership and the CCHP Governing Board. The Director will build and enhance relationships with partner agencies along with medical and behavioral health agencies in support of the CCHP program.

Other Key Responsibilities:

  • Represent the program to the agency, provider partners and external organizations.
  • Identify and participate in Community and Agency based opportunities to increase referrals and program awareness.
  • Facilitate the monthly CCHP Governing Board meetings.
  • Ensure program operates in compliance with contractual obligations, service delivery standards, and adheres to agency policies and procedures.
  • Lead the centralized staff team of directors and administrative positions by providing regular supervision, performance appraisal and supporting ongoing professional development.
  • Develop and monitor key performance indicators to monitor and manage the overall CCHP (BH and LTSS) budget and staffing requirements.
Qualifications:
  • Master's degree in a health, behavioral health or human services related field, required. Licensed Practitioner of the Healing Arts (LICSW, LCSW, LMHC, LADAC I), highly desired.
  • A minimum of five years of related managerial experience in a supervisory capacity, required.
  • Applicable knowledge and experience in health, behavioral health, managed care and human services continuums.
  • Highly organized with strong management skills and ability to evaluate and control complex delivery systems.
  • Data literacy and knowledge of ACO/MCO, highly desired.
