ESSENTIAL DUTIES AND RESPONSIBILITIES: To perform this job
successfully, an individual must be able to perform each essential duty satisfactorily.
The requirements listed below are representative of the knowledge, skill, and/or

1. Personnel
a. Ensure all program supports and services are delivered while on shift.
b. Ensure all program staff maintains expected levels of participation in
program operations.
c. Provide on the job training and modeling for all employees, and direct
the activities of employees while on shift.
d. Provide leadership that supports positive work environment and
employee morale.
e. Provide input to probationary and annual performance evaluations for
all program staff.
f. Provide input regarding the development of all programs staff and
coordinate training needs to address identified need areas.
2. Facilities
a. Assure that the home maintains a pleasant physical appearance and
that items in the home represent the interests and preferences of the
individuals supported.
b. Assure home maintains compliance with local building codes, Survey
and Certification mandated regulations and individuals needs
identified in the house Safety Plan.
3. Quality Assurance
a. Assure that ISP goals and objectives are followed through with and
required documentation exists for each goal area.
b. Communicate effectively with House Manager and Clinical staff to
assure that all needed supports are available to each consumer’s area
of interest and/or need.
c. Evaluate the unique support needs not included within the ISP process
and utilize all resources delivered to address these needs.
d. Maintain a flexible work schedule to insure appropriate levels of
management oversight.
e. Attend all required meetings.
f. Monitor consumer entitlement and spending monies using generally
accepted accounting practices.
g. Assure staff decisions are based upon consumer choice and
empowerment.
h. Ensure consumer meetings are held monthly and documented
i. Develop community schedule with input from those served. Ensure all
consumers are actively involved in the community
j. Assist with the coordination of health care needs.
k. Familiar with and compliant with ass aspects of Community Rule
Indicators
l. Follow all procedures as outlined in the standards manual
4. Administrative
a. Assure that all required documentation is completed within a timely
fashion and according to established guidelines
b. Communicate timely and accurately with all ancillary departments.
c. Contribute to maintaining high level of employee morale.
d. Responsible for the outcome of the Survey and Certification Review.
e. maintain open and effective communication with DDS Service
Coordinators, parents, guardians, employees, other managers, vendors,
and community.
f. Assist manager with program schedule
g. Effective communication of all program needs reported to the House
manager in a timely manner.
h. Report any suspicion of abuse or neglect to DPPC as a mandated
reporter.
i. Attend all required meetings.
j. Provide management oversight during inclement weather.

QUALIFICATIONS AND REQUIREMENTS:
• Maintenance of an acceptable CORI and fingerprinting
• Negative Drug Screening
• Valid driver’s license
• Use of personal vehicle for work purposes
• Maintain flexibility with weekly scheduled hours in order to accommodate program
needs
• Demonstrated commitment to the philosophy of community inclusion

EDUCATION and/or EXPERIENCE:
• Associates Degree in Human Service field with one year’s experience or three years
experience in Human Services.
• Demonstrated history of quality work performance and teamwork in previous
positions.
• Proven Leadership skills

EQUIPMENT/MACHINES USED IN THE PERFORMANCE OF THIS JOB
• The incumbent must maintain skills necessary to safely and efficiently operate
equipment and machines used in the performance of this job.
