At OhioGuidestone we aim to plant the seed of wellness in the lives of everyone we interact with. Twenty-four hours a day, seven days a week, we provide crisis intervention services and a comprehensive array of behavioral health services so that those in need are not caught trying to piece together services on their own. Our new Certified Community Behavioral Health Clinics are uniquely positioned to serve as the cornerstone in our state’s new crisis response system, decreasing the burden on first responders and emergency medical services while cultivating a sense of vitality and stability in our clients and in the communities with service.
Crisis Navigator will be doing important and meaningful work that will bridge the gaps in SUD, Mental health, and physical healthcare to improve and save lives. Based on a person and family-centered plan of care, the Crisis Navigator will be the pillar of our crisis management services, stabilizing adults and children in their time of need until their needs can be met by our long-term care clinicians. The CNT position will be providing social services, including behavioral and mental health assessmentssocial determinants to health and wellness and educational systems advocacy, as necessary to facilitate the wellness and recovery of the whole person.

Essential Functions:

  • Demonstrate the values and guiding principles of Our Promise each day, performing with the highest standards of excellence, integrity and ethics.
  • Respond to emergency or crisis-related requests from individuals and/or agencies and provide appropriate care and services.
  • Conduct comprehensive clinical screens, and assessments as part of the intake and diagnostic process, ongoing provision of services and/or crisis intervention.
  • Develop and provide complex client-centered treatment and intervention plans based on evidence-based practices that addresses the clinical needs of individuals in single or multiple areas of focus.
  • Network with community partners to research and coordinate a variety of resources and services to address comprehensive, intensive or long term individual needs.
  • Complete and submit clinical documentation regarding services provided; evaluate and document client’s progress; monitors the quality and effectiveness of services provided.
  • Provide outreach, education, and training to staff, partner agencies, and community, to promote OhioGuidestone programs and services.
  • Work as part of a multi-disciplinary team and maintain effective working relationships with law enforcement, criminal justice professionals, social service agencies, and any other organizations involved in the delivery of services.
  • Demonstrate positive leadership, promote an inclusive work environment and present the Agency in the most positive light with all internal and external contacts.
  • Exhibit positivity, flexibility and a willingness to take on new responsibilities as requested or required.

Education & Experience:

  • Bachelors; Degree in Social Work, Counseling, or other related Human Services field. Master’s Degree preferred.
  • Current licensure in the state of Ohio (LSW, LPC or MFT)
  • 3 year of experience in behavioral health care, working with individuals with mental illness and/or substance use disorders.
  • Strong knowledge base of community providers and resources to include behavioral and physical health care, housing, transportation, employment/academic supports and other social service agencies.

Qualifications:

  • Effective oral and written communication skills.
  • Ability to provide client-centered, evidence-based clinical interventions.
  • Ability to maintain clarity of thought and take action in emergency situations.
  • Ability to assess and organize complex information regarding a client needs to ensure appropriate provision of care.
  • Effective interpersonal skills to communicate and encourage collaboration among therapeutic team members.
  • Ability to travel to assigned sites and/or transport clients as requested or required.
  • Bi-lingual: Spanish-speaking, reading, writing skills preferred.
  • Ability to work assigned shifts to provide coverage for a 24/7 program, including holidays and weekends.
