Are you a kind, passionate individual who thrives in supporting your patients? Are you looking to work on a collaborative care team that supports the health and well-being of a vulnerable population? Penobscot Community Health Care is seeking a Family Nurse Practitioner or Physician Assistant to join the Hope House Health and Living Center in Bangor, Maine. This is an opportunity for an advanced practice provider that has a unique skillset and interests that include—community outreach, wound care, substance use disorder treatment, and a passion for working with patients who are navigating socioeconomic challenges.

  • The position is part-time, 20-30 hours per week, with a Monday-Friday schedule, specific days worked to be determined with candidate. No nights, no weekends.
  • Adult Primary Care patient panel focus, open to all, with strong emphasis on Substance Use Disorder. 
  • Practice located on campus with 54-bed shelter, and 48-bed transitional housing center.
  • Experienced, interdisciplinary team includes psychiatrists, family medicine physicians, psychotherapists, and licensed clinical social workers.
  • Practice offers full-range of integrated outpatient primary care services including mental health, recovery treatment and medication assisted treatment.
