We are looking for a Licensed Social Worker to join our team. As a full time, Licensed Social Worker, you are responsible for providing support and assistance to residents and their families in a long-term care setting. You will work closely with the interdisciplinary team to assess and address the social and emotional needs of residents, develop care plans, and provide counseling and support to residents and families.
Responsibilities:
  • Assess and address the social and emotional needs of residents
  • Develop care plans in collaboration with the interdisciplinary team
  • Provide counseling and support to residents and families
  • Advocate for residents and their families
  • Coordinate discharge planning and community resources
  • Participate in interdisciplinary team meetings and resident care conferences
  • Maintain accurate and timely documentation
  • Participate in quality improvement initiatives
Requirements:
  • Bachelor's degree or master's degree in social work, or related field, including but not limited to, Sociology, Psychology, or Rehabilitation Counseling
  • Licensed Social Worker in the state of Michigan preferred
  • Experience working in a long-term care setting preferred
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong organizational and time management skills
  • Proficient in Microsoft Office and electronic medical records
