We are seeking a compassionate and skilled RN Case Manager to join our Hospice team. The RN Case Manager will be responsible for providing exceptional care to an assigned group of patients and their families, ensuring their needs are met through the coordination of services by the Interdisciplinary Team. This role is pivotal in delivering care consistent with federal and state regulations, hospice policies, procedures, and standards of practice. The Regional Hospice office serves as the home base, covering six counties, with limited travel to the outer reaches of these counties. Key Responsibilities: • Deliver comprehensive nursing care to hospice patients and their families based on the nursing process. • Coordinate and oversee the services provided by the Interdisciplinary Team, ensuring a holistic approach to patient and family care. • Develop, implement, and evaluate individualized care plans for patients. • Monitor patient conditions and report changes to the physician and other team members. • Provide education and support to patients and families regarding the hospice process and end-of-life care. • Ensure all care provided is in compliance with federal and state regulations, hospice policies, and standards of practice. • Maintain accurate and timely documentation of patient assessments, care plans, and interventions. • Participate in Interdisciplinary Team meetings to discuss patient care and coordinate services. • Serve as a liaison between patients, families, and healthcare providers to facilitate communication and care coordination. • Travel within the assigned six-county region, primarily avoiding the outermost areas. Qualifications: • Graduate of an accredited school of nursing (NLN accredited); BSN preferred. • Current RN license in Wisconsin (WI) and Illinois (IL), or ability to obtain both. • Valid driver’s license and reliable transportation. • Experience in hospice or palliative care preferred. • Strong understanding of the nursing process and end-of-life care. • Excellent communication and interpersonal skills. • Ability to work independently and as part of a multidisciplinary team. • High level of empathy, compassion, and professionalism.
