General Description of Position:

Our Care Navigators serve as a unique resource and helping hand for our patients, providing one-on-one expert care throughout their surgery and treatment journey. Acting as the patient’s advocate, communication hub, and guide, this role requires exceptional skills in planning, organizing, and coordinating patient care and managing day-to-day navigation activities.

Essential Duties and Responsibilities:

1. Care Coordination:
   • Review daily logs and notes from clinical records and reports to ensure continuity of care, proper documentation of the nursing process, appropriate interventions, and timely documentation.
   • Collect and track data to improve outcomes, including data entry into required systems.
   • Coordinate care as the single point of contact/resource for patients, staff, and physicians, ensuring all team members have current information.
2. Patient Interaction and Advocacy:
   • Provide expertise as a professional nurse or social worker with orthopedic knowledge.
   • Offer care navigation services throughout the continuum of care and link patients with necessary resources when gaps are identified.
   • Act as a patient advocate and educator, available to patients and their families before, during, and after surgery.
   • Provide telephonic and in-person care navigation services, focusing on delivering an exceptional patient experience.
3. Problem Solving and Reporting:
   • Report any problems or concerns to the appropriate staff and follow up until resolved.
4. Miscellaneous:
   • Perform any other related duties as required or assigned.

Qualifications:

• Education and Experience:
  • Specialized knowledge in a relevant field, such as basic accounting or computer skills, equivalent to two years of college or specialized training plus three years of related experience and/or training. An equivalent combination of education and experience is also acceptable.
• Certificates, Licenses, Registrations:
  • Required: Current LPN/RN or Social Work (BSW or MSW) license.
  • Preferred: Not indicated.
• Software Skills Required:
  • Intermediate: Word Processing/Typing.
  • Basic: 10-Key, Database, Spreadsheet.
