Description

The Behavioral Health Counselor (ACT Team) position works as a member of a multi-disciplinary Assertive Community Treatment team serving transition age youth in the Multnomah County Homeless Youth Continuum.

With Master's degree: $59,680.61 to $65,876.22

With Licensure: $65,642.30 to $72,456.82

Essential Duties

  • Participate on interdisciplinary team providing community-based, wrap-around support.
  • Participate in outreach activities necessary to offer services and engage participation in a non-coercive manner.
  • Provide initial comprehensive assessment and re-assessments of participants, including mental status exam, DSM diagnosis, and clinical formulation.
  • Facilitate development of a Service Plan with participants.
  • Recommend services and supports by signing off on Service Plans written by the employee or other team members within 10 business days of the start of services.
  • Execute treatment services consistent with professional judgement and licensure as identified on Service Plan, including individual and group counseling and skills interventions.
  • Collect and utilize treatment outcomes related to behavioral health counseling.
  • Participate in admission and discharge planning for any participant needing hospitalization or subacute level of care.
  • Participate in crisis planning and crisis intervention, which may include after-hours work.
  • Participate in the rotation of carrying after-hours crisis phone and providing holiday coverage with other members of the team/program.
  • Provide advocacy and assist participants in accessing the services and other resources they need.
  • Coordinate access to shelter and housing services.
  • Ensure timely documentation of services and outcomes.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to Outside In policies and agreements regarding confidentiality, privacy, and security.
  • Other duties as assigned.

POSITION SPECIFIC BENEFITS:

· Clinical supervision towards LCSW or LPC licensure and financial support accessing testing and training

· Supervision towards CADC or MAC

Qualifications


Knowledge and Skills

Knowledge

  • Through knowledge of relevant treatment models including Assertive Community Treatment (ACT), Integrated Dual Diagnosis Treatment (IDDT), and Individual Placement and Support (IPS).
  • Knowledge and understanding of adolescent development, Trauma Informed Care, Stages of Change Theory, harm reduction and Motivational Interviewing.

Skills

  • Strong organizational skills, attention to detail, and focus on data collection.
  • Ability to work with a variety of clients and diagnostic categories.
  • Ability to work with a diverse population and interact effectively with different types of people.
  • Good communication and conflict resolution skills.
  • Ability to work both independently and as a member of a team.
  • Ability to support agency goals and the operational functioning of the Behavioral Health Department.
  • Ability to complete all required documentation and information input in a professional, thorough, and timely manner.
  • Commitment to continual learning and quality performance.
  • Ability to maintain professional boundaries and performance at all times.
  • Ability to navigate technologies used in this position including Credible Behavioral Health Electronic Health Record, MS Office Applications, and Windows Server.  
  • Valid driver’s license.
  • Ability to be insured to drive participants in agency vehicle and car share.
  • Spanish language fluency preferred.

Education and Experience

  • Oregon LPC or LCSW.
  • Master’s degree in a human services field and meet State guidelines for QMHP required.
  • At least two years’ experience in mental health counseling required.
  • Additional credential in Chemical Dependency counseling equivalent to CADC I preferred.
  • Dual diagnosis counseling experience preferred.
  • Experience with relevant treatment models including Assertive Community Treatment (ACT), Individual Placement and Support (IPS), and Integrated Dual Diagnosis Treatment (IDDT) preferred.

Working Conditions

This is a AFSCME Union represented position.

This job includes working in a standard office environment as well as a variety of community locations to offer community-based treatment. This means that the employee will meet with participants in community locations which may include the participant’s home, hospital, and other surrounding community settings. The employee will be responsible for transporting participants in a rented vehicle.

Physical Requirements

  • Must pass pre-employment background check. This includes clearance by the DHS Background Check Unit.
  • Standing, sitting, walking, on a regular basis.
  • Driving, climbing, balancing, reaching, kneeling, crouching, stooping and lifting up to 25 pounds on a frequent basis.
  • Must be able to climb stairs several times a day.
  • Regular use of hands and fingers to type.
  • Talking and hearing is required on a regular basis.
  • Availability for after hours and holiday coverage rotation.
