Palatka Center for Rehabilitation and Healing is now hiring an experienced MDS Care Plan Coordinator. The MDS Coordinator can be a LPN or RN.

MDS Coordinator Summary Description:

The MDS Coordinator underthe directionof the Administrator or Directorof Nursing, is responsible for coordinating the Resident Assessment Instrument (RAI)process including completion of an accurate Minimum Data Set (MDS) anddevelopment of the Interdisciplinary Plan of Care. Additionally, thisindividual will lead the daily Medicare Prospective Payment System(PPS)/Medicaid Case mix Meeting. These duties are to be performed in a timelymanner consistent with Centers for Medicare & Medicaid (CMS)guidelines and policies and procedures. This individual also establishes andmaintains positive relationships with all members of the healthcare team.

EssentialFunctions and Responsibilities:

1. Maintains current MDSstatus of each patient/resident, in accordance with all state and federalguidelines, including the Omnibus Budget Reconciliation Act (OBRA), MedicarePPS and any Medicaid payment system, as applicable.

2. Schedulespatients/residents for appropriate OBRA, Medicare PPS and/or Medicaid required assessments.

3. Communicatesschedule to all other departments and families, as required.

4. Monitors clinicalsystems for changes in patient/resident condition.

5. Responds topatient/resident change in condition by coordinating patient/residentre-assessment and re-evaluation plan of care.

6. Strategically setsassessment reference date (ARD), in conjunction with the

interdisciplinaryteam, to capture needs, care and services delivered to most accurately reflectthe status of the patient/resident.

7. Accurately completesportions of MDS and CAAs as assigned, in accordance.

with policies,procedures, and guidelines.

8. Assures compliancewith federal, state, Medicare and Medicaid regulations, and facility policiesand procedures as related to completion of an accurate electronic submission ina timely manner and coordination of the RAI process.

9. Coordinates anddevelops plan of care as required.

10. Monitors MDS and careplan documentation on all patients/residents, assuring that complete, orderlyand chronological documentation is maintained in the file.

11. Maintains MedicareCertifications /Re-Certifications

12. Participates in QAPI/QM process

13. Maintains proficiencyin software programs for MDS completion and is able to access and utilizeappropriate reports.
