Palliative Care Alliance is a fast- growing company. We are seeking an RN or LPN to join our team and provide palliative care home visits to our patients. We currently have an opportunity in WEST valley. This is a great opportunity for the right person. Seeking a self-starter who enjoys working directly with patients in their place of residence.

• Flexible schedule-You control and make your own schedule
• Provide supportive care for patients/Patient teaching
• We are Community based palliative care, so you will be seeing the patients in their home setting (Private home, Assisted Living, IL, Group Home)
• Coordinate care needs with other healthcare providers
• Orientation training and great ongoing office support.
• Very user friendly charting system
• Attend monthly ZOOM IDT meetings
• Great compensation and plenty of opportunity to grow with the company

Pay: From $60.00 per hour
