Job Summary:

Provides personal care services under the direction of Registered Nurse. A Home Health Aide is assigned to specific clients by the Registered Nurse or other appropriate clinician and performs services for clients as necessary to maintain their personal comfort.

Responsibilities:

• Drives to prospective patients’ residences.
• Ensures quality and safe delivery of home health services, including education and information to patients, families and caregivers pertinent to diagnosis and plan of care.
• Implements plan of care initiated by the case manager
• Provides accurate and timely documentation consistent with the plan of care
• Assist ailing patients in daily necessary activities such as eating, bathing, etc.
• Participates in coordination of home health services
• Uses equipment and supplies effectively and efficiently.
• Performs other duties as assigned by the RN.
• Perform duties consistent with the Nursing Practice Act including the Standards of Competent

Qualifications:

• Hold a current Certified Nursing Assistant License (CNA) in the State of Arizona.
• Home health care experience preferred.
• CPR certification.

Job Requirements:

• Must have a valid driver’s license, and auto insurance.
• Required to present a negative TB test and other tests as required by agency policy.
• Must be able to do fieldwork.
• Must have excellent written and verbal skills.
• Excellent planning, organization, and presentation skills.
• Ability to think and work independently and with direction and communicate with staff members in a fast-paced and sometimes stressful environment.
• Must be in good health and be able to fulfill the physical requirement of the job.
• Must be able to lift a minimum of 25lbs.
• Must be willing to drive all over Arizona.
