Care Coordinator
Job Overview:
We are seeking a dedicated Care Coordinator to join our team. The Care Coordinator will be responsible for overseeing and coordinating care services for individuals in need. This role requires compassion, attention to detail, and the ability to work collaboratively with a multidisciplinary team.
Responsibilities:
- Coordinate and schedule care services for individuals requiring assistance
- Conduct assessments to determine clients' needs and develop personalized care plans
- Monitor and evaluate clients' progress and adjust care plans accordingly
- Communicate effectively with clients, families, and healthcare providers
- Provide direct support to clients with activities of daily living
- Ensure compliance with all relevant regulations and standards
- Assist with meal preparation and medication management
- Monitor vital signs and report any changes to the healthcare team
- Specialize in senior care, dementia care, long-term care, and memory care
- Provide first aid as needed in emergency situations
Requirements:
- Previous experience in a similar role preferred
- Knowledge of care coordination systems and procedures
- Certification in first aid and CPR
- Familiarity with nursing home environments
