Pathways Healthcare is Hiring a Hospice Clinical Manager Near Londonderry, NH

Position Details:
  • Main Office location: Londonderry, NH
  • Job Type: Full Time
Responsibilities:
  • Coordinate and supervise all direct and indirect patient services provided by clinical personnel in the organization.
  • Offer guidance and counseling to coordinators and Clinical Supervisors, assisting them in continually enhancing hospice care services provided by the organization's personnel.
  • Assist clinical personnel in assessment, planning, implementation, and evaluation of patient and family/caregiver care.
  • Interpret operational indicators to detect census changes and volume fluctuations that could impact staffing levels, revenues, or expenses.
  • Collaborate with Human Resources to hire, evaluate, and terminate organization personnel.
  • Conduct annual evaluations of clinicians, or more frequently as required.
  • Oversee the maintenance of patient clinical records, statistics, reports, and records to comply with local, state, and federal laws and regulations.
  • Responsible for maintaining adequate and appropriate inventory supplies and equipment for providing patient services.
  • Develop, implement, and evaluate orientation programs for new organization personnel. Plan and conduct in-service and continuing education programs to meet the needs of the team.
  • Assist in evaluating organizational performance through performance improvement programs, productivity, quarterly and annual reviews, ensuring the quality and safe delivery of hospice services.
  • Recommend and administer organization policies and procedures, while staying informed about changes in nursing and hospice care and sharing information with relevant personnel.
  • Actively promote hospice referrals in the healthcare community.
Qualifications:
  • Registered nurse in the State of [STATE HERE] with current licensure to practice professional nursing in the State.
  • Bachelor’s degree in nursing from an accredited program by the National League for Nursing. Master’s degree in hospice preferred.
  • Three years of management experience in a hospice or related health care organization is desirable.
  • Demonstrated ability to supervise and direct professional and administrative personnel.
  • Ability to market and deal tactfully with customers and the community.
  • Maintain current BLS CPR certification.
  • Possess a valid driver's license and own a reliable, insured vehicle.
  • Has excellent observation, verbal, and written communication skills.
  • Knowledge of business management, governmental regulations and ACHC standards.
