Pathways To Housing DC is Hiring a Senior Case Manager Housing Program Near Washington, DC

Pathways to Housing DC (Pathways DC) is based in Washington, DC and serves people in the District and Montgomery County, MD. We are an innovative and nationally recognized nonprofit committed to ending homelessness for individuals with complex health challenges. Pathways DC is one of the originators of the “Housing First” model to end homelessness, in which services are provided without conditions and pre-housing requirements such as curfews, mandated sobriety or compliance with medication. We believe housing is healthcare and that housing is a basic human right. Pathways provides an array of services including housing, street outreach, case management, drop-in services, and behavioral health support. Our model is based on client choice and the idea that the people we serve are the architects of their recovery!

The Permanent Supportive Lead Case Manager has responsibility for supervising 3 Case Managers and managing a caseload of 9 clients.

The Lead Case Manager is responsible for the daily operation and clinical supervision of 3 Case Managers of the Permanent Supportive Housing Program (PSHP). The Lead Case Manager is responsible for assuring that the agency values and philosophy of consumer choice, harm reduction and respectful concern are expressed in all aspects of team practice. The Lead Case Manager maintains oversight to ensure that client self-identified goals for recovery, rehabilitation and community integration are supported by appropriate services and best practices. 

Lead Case Managers are members of the Permanent Supportive Housing Program (PSHP) who provide flexible, community based services that are designed to promote the stability, recovery and community reintegration of formerly homeless individuals assigned to PTHDC by DC’s Department of Human Services. The Lead Case Manager maintains a primary caseload of 9 clients and provides case management services in a compassionate and responsible manner.

Note: All Pathways employees must be vaccinated against COVID-19 as a condition of employment, unless there is an approved medical or religious exemption. All new employees will be required to present a COVID vaccination card or an approved exemption request prior to starting employment. Unvaccinated employees will be subject to weekly COVID testing.

SUPERVISOR RESPONSIBILITIES:

  • Supervise and facilitate the professional development of the Case Managers

  • Collaborate with the Clinical Supervisors and Program Manager in determining staffing needs and interviewing potential Case Managers, as well as assigning caseloads to Case Managers

  • Train, supervise and evaluate the Case Managers in conjunction with the Clinical Supervisors and Program Manager

  • Plan and implement all client services, in collaboration with the Case Managers

  • Supervise electronic and written record-keeping activities, e.g., progress notes, service plans, client budgets, incident reports and client charts

  • Coordinate services of the PSHP with other inpatient and outpatient community treatment, support and social services 

  • Collaborate with the Program Manager regarding housing issues of PSHP clients

  • Represent PSHP in internal meetings. May represent Pathways on external related meeting in conjunction with the Clinical Supervisors and Program Manager

  • Coordinate the use of agency vehicles designated for use by the PSHP

  • Perform other related duties as assigned

CASE MANAGER RESPONSIBILITIES: 

  • Collaborates with client in the formulation of a Case Management Plan that is reviewed and modified with client on a regular basis; 

  • Provides outreach, case management, counseling, advocacy, and other needed services to clients in any environment including: the streets, shelters, jails, hospitals, apartments, office, etc; 

  • Provides individually tailored services to each client in the following areas: housing, school, training & work opportunities, activities of daily living, health, wellness self-management, relapse prevention, money management & entitlements, medication support, self help & empowerment, problem solving, family life & social relationships and integrated treatment for substance abuse; 

  • Maintains written and computerized records, compile reports and completes other program documentation (e.g. progress notes, case management plans, intakes, biopsychosocial assessments, monthly visit reports, incident reports); 

  • Responsible for the monthly client contact requirements as follows: 4 contacts per month for homeless clients, including 2 face to face contacts; 2 contacts per month for housed clients, including 1 face to face contact in the client’s apartment; additional monthly contacts as determined by the Case Management Plan and assessment of client needs; 

  • Coordinates and monitors referrals to community services and advocates client participation;

  • Participates in monthly staff meetings;

  • Participates in staff trainings as scheduled; 

  • Performs related work as assigned;

QUALIFICATIONS:

  • Master’s Degree in Social Work, Mental Health Counseling or related field from an accredited College or University.

  • DC licensure as a Licensed Graduate Social Worker (LGSW) or Licensed Graduate Professional Counselor (LGPC) IF APPLICABLE

  • 2 years’ experience in case management and/or counseling services to individuals/families experiencing homelessness or other related populations. 

  • Candidates with a BA in the Human Services field with 4 years of experience working in permanent supportive housing may be considered. 

  • Experience supervising staff performing case management services strongly desired.

  • Excellent interpersonal, communication and writing skills, as well as computer literacy.

  • Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other minority populations.

  • Bilingual English/Spanish strongly desired.

  • Driver’s license and access to motor vehicle required
