Position Summary: This position is responsible for providing outpatient rehabilitation services in an in-home setting for the older adult population. The in-home setting could be a patient's private residence or in a patient's room in a SNF/Dementia-care facility, in an assisted living facility, or in an independent living facility. This position will evaluate and treat in the most functional environment which is the patient's home and assesses the need for, develop, and deliver physical therapy interventions to facilitate rehabilitation both on an intermittent and maintenance-based scale. The Physical Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients. The Physical Therapist will implement individualized therapy program designed to restore, reinforce, and enhance performance and optimize independence. In addition, this position instructs, educates and trains patients and caregivers in those skills and functions essential in promoting quality, independence, and productivity, in order to diminish or correct movement disorder pathology.
Essential Functions:
o Therapeutic exercise including aerobic capacity conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuro-motor development training, relaxation, strength, power, and endurance training
o Functional training
o Manual therapy
o Airway clearance techniques
o Integumentary repair and protection
o Use of electrotherapeutic, physical agent and mechanical modalities
o Education on Chronic Disease Management
o Falls Assessment and Interventions to reduce fall risk
o Case Management in the home and community
