PCC Community Wellness Center is a network of 11 health centers that offer comprehensive primary care and support services in Chicagos west side and the near west suburbs. PCC operates as a medical home, where we offer comprehensive primary care with the integration of behavioral health and oral health care in order to address the multifaceted elements of individual health.
