Summary:
  • The manager of care coordination provides leadership and strategy, for the case management department. With responsibility for the case management operations and staff, this position oversees resource management, coordination of care and transition/discharge planning for all assigned patients. A thorough understanding of patient flow, hospital thru put and potential discharge planning barriers is a must. An ability to identify and implement solutions is required. In addition to discharge planning the care coordination manager possesses an understanding of fiscal accountability, payer contracts, value based quality care and social determinants of health. Well versed in CMS and TJC standards, this position cultivates relationships with service line leaders in order to lead staff. A working knowledge of the EMR, documentation standards work flow processes and budgeting is required. The manager collaborates closely with the social work team staff to ensure patients experience a seamless transition in care and ensures hospital reimbursement.
Responsibilities:
  • Responsible for daily operations such as hiring, orientation and training, mentoring and assessing productivity and work flow. Provides feedback to staff in the form of quality reviews and performance appraisals ensuring targeted performance levels. Organize daily coverage plan including call outs, and scheduled time off, off hours and weekend schedules. Assumes responsibility for 24/7 coverage in assigned areas. Continuously monitor methods to improve effectiveness in the discharge planning process and transitions in care across the continuum. Assists with pathway development and data analysis. Works with management team and service line leaders at all entities to develop discharge planning/transition solutions and maintain an appropriate LOS for patients. Represents Case Management division on hospital and system wide committees. Accountable to develop the budget for assigned AU’s and maintain fiduciary responsibility. Analyze department data and plan for program improvements, including the development of software applications and technology. Analysis of denials and appeals and develops work plans to improve revenue cycle functions.
Credentials:
  • Registered Nurse - PA (Required)
  • Pennsylvania RN License required.
Education or Equivalent Experience:
  • Master of Arts or Science (Required)
  • Education Specialization: Health Administration/Health Related Field Equivalent Experience:
  • 5 years experience with Nursing Management
