COMMUNITY HEALTH WORKER, PHCS
REPORTING/COORDINATING RELATIONSHIP: Executive Director
SUPERVISORY RESPONSIBILITIES: None
PRINCIPLE RESPONSIBILITIES:
Community health workers are culturally competent and effective community leaders. They are local employees trained to act as liaisons for the uninsured, underinsured and underserved population in their communities. Community health workers work within their own communities to bridge the gaps and empower individuals to have a healthier quality of life.
Duties include:
SILLS AND KNOWLEDGE REQUIRED:
This position requires: the ability to disseminate information; the ability to follow through and carry out plans of action; and good verbal skills. Priority will be given to those applicants who have been consumers of the social, housing, environmental, health or mental health system, or who have worked closely with these systems of care. (Experience preferred. Although a degree is not required in Social Science field, applicants with a degree will also be considered.)
MINIMUM QUALIFICATIONS:
High School Diploma or equivalent. Must have a valid driver’s license, reliable transportation, and be willing to transport clients on a daily basis. Must present proof of automobile insurance and pass a Criminal Record Check. Must present proof of a recent (less than 1-year-old) or 2-step TB skin test.
