Summary: Provides primary care nursing, patient education, health care maintenance, patient counseling, and normative care services to patients at Peoples Community Health Clinic. Works with providers and administers nursing care utilizing the nursing process (assessment, planning, implementation, and evaluation). Assists with the planning, development and implementation of health education programs and materials, and provides assistance to and supervision of the Patient Clinical Representatives in areas of nursing beyond their scope of practice. Performs all defined services and other related duties in accordance with the mission of Peoples Community Health Clinic.

Essential Functions:

• Provides primary care for patients.
• Provides normative care for patients.
• Provides patient education.
• Provides patient counseling.
• Coordinates the referral process by initiating prior authorizations when required.
• Protected Health Information Access (HIPAA) Patient Records – Yes Medical Information System - Yes
• Supports a service-oriented atmosphere in accordance with Peoples Community Health Clinic Mission and Philosophy.
• Works to improve work processes and clinical outcomes including health disparity and quality improvement collaboratives.
• Follows policies and procedures.
• Develops and maintains own competence.
• Maintains a safe working environment and practices safe working habits.
• Assists in control of Peoples Community Health Clinic resources.

Qualifications:

Education: Graduate of an accredited practical nursing program and current Iowa LPN license
