We are currently seeking a dedicated and passionate Case Manager with a background in Senior Living.
Case Manager Responsibilities include:
- Orient and assist each resident to adjust to life in the community
- Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to our community, including their social, emotional, and psychological needs
- Assist each resident to maintain family and community ties and to develop new ones
- Complete quality management reviews to ascertain if the department is meeting state regulations and PSL policies and follow up as needed
- Work with the care department and others to promote and protect resident rights and the psychological well-being of each resident
Case Manager Education & Experience:
- Bachelors degree with major course work in human services AND one year of full-time work experience in a related field OR
- Associates degree with major course work in human services AND three years of full-time work experience in a related field required
- Case management experience required
- Memory Care/ Assisted Living experience required
- Compassion for and desire to work with the elderly
