The responsibilities for the Assessment and Referral Clinician include the following:
•Receive referral calls, obtain minimal demographic information, and refer the call to the appropriate source or schedule an assessment.
•Process all requests for information and external referrals.
•Complete pre-certifications as needed.
•Assist in arranging admission of patients and communicating the admission to the other needed departments including business office, medical staff, and nursing.
•Determine the needed Releases of Information and process them accordingly, per policy. Assist in obtaining needed records and information.
•Assess, diagnosing and treatment a variety of patient populations.
