Required Skills: Demonstrated ability to recognize symptoms, identify treatment alternatives and recommend preventive health care measures. Required Experience: Doctor of Dental Medicine (DMD) or Doctor of Dental Surgery (DDS) from a pre-doctoral dentistry program accredited by the Commission on Dental Accreditation of the American Dental Association or the Commission on Dental Accreditation of Canada. Minimum qualifications include an applicable State License. Washington and Oregon providers must have a valid DEA number, current CPR certification and 1 - 3 years practice experience, or GPR or AEGD training.
