About the Role:

The Director of Care Management provides support to the care management team across the health system. The Director will participate in strategic initiatives set forth by the organization, leading all care management activities, ensuring patient care coordination and adherence to clinical protocols and regulatory standards. In conjunction with the CFO and other Executive Leaders the Director will inform and execute the strategic direction that aligns with the long term vision and goals of Phelps Health. The Director will be responsible to develop, implement and monitor all day-to-day operations and leadership of employees in the Care Management department.

Responsibilities:

  • Lead all care management activities, ensuring patient care coordination and adherence to protocols and regulatory standards
  • Develop and implement care management programs, management protocols, policies, departmental strategy in consultation with senior leadership
  • Monitor and coach staff to meet expected performance expectations
  • Create a cohesive environment promoting teamwork and collaboration
  • Oversee and develop quality monitoring programs and work collaboratively with the Quality and Clinical teams to achieve and sustain improvement goals.
  • Conduct data analysis and reporting as required.
  • Perform internal audits to ensure compliance with organizational, state and federal practices, policies and regulations
  • Manage departmental budget and staffing
  • Serves as a liaison between community partners and the organization in regards to discharge planning, health equity, and care management practices.
  • Participates collaboratively in required committees
  • Continuously monitors for compliance with discharge planning, conditions of participation and patient rights, outpatient regulatory compliance related to SDOH, TCM/CCM billing practices and collaborates with department directors to implement changes.

Qualifications:

  • Two years of case/care management experience preferred in outpatient and/or inpatient setting
  • Prior managerial experience in a healthcare setting

Education details

  • Bachelor’s degree in a related field
  • Certifications: RN or MSW preferred

Pay range and compensation package:

$106k-$160k
