Public Health Management Corporation is recognized by the Health Resources and Services Administration (HRSA) and the State of Pennsylvania as a Patient-Centered Medical Home (PCMH). The Agency for Healthcare Research and Quality’s (AHRQ) defines Patient Centered Medical Home (also known as Primary Care Medical Home) as primary care delivery model provides patient-centered, comprehensive, accessible, high-quality, efficient, and coordinated services. Managed Care Organizations (MCOs) provide health centers funding and oversight to ensure they remain in compliance with these principles and standards.

The PCMH Manager will service as a champion all Patient Centered Medical Home work. In this capacity, they will provide operational oversight and grants management for the Health Network’s Patient Centered Medical Home program. The PCMH Manager will also collaborate with other program managers and supervisors across the PHMC Health Network in an interdisciplinary effort to fulfill and enhance PCMH standards. The Patient Centered Medical Home (PCMH) Manager will additionally directly supervise Health Network Community Health Workers and community-based care management staff, as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The PCMH Manager will provide grants management oversight for all Patient Centered Medical Home funding sources and will ensure compliance and fidelity to all federal, state, and local PCMH standards. They will also work to ensure the operational and programmatic success of PCMH-related programs. The Patient Centered Medical Home Manager will develop, lead, and ensure the ongoing success of all PCMH activities. These activities include, but are not limited, to the following:

· Represent the Public Health Management Corporation health centers in PCMH-related forums.

· Analyze and use PCMH-related health outcome data to inform care management strategies and initiatives.

· Develop and implement training for all Public Health Management health center staff on PCMH principles and standards, as required by the Joint Commission.

· Keeps abreast of all pertinent federal, state, local PCMH-related and PHMC regulations, laws, and policies as they presently exist and as they change or are modified.

· Identity, establish, and maintain community partnerships to better meet the needs of patients.

· Report to internal and external stakeholders on program and health center outcomes.

· Provide thought leadership to around community-based care management, community health work, and social determinant of health initiatives.

· Organize and participate in community engagement and education events, as needed.

The PCMH Manager will also provide director supervision to Health Network Community Health Workers and community-based care management staff, as needed.

· Hire, supervise, and ensure high-quality performance of community-based care management staff.

· Conduct annual evaluations and create performance improvement plans for direct reports.

· Comply with all PHMC policies and procedures, including all HIPAA and Confidentiality policies, as they apply to the job.

· Understand and adhere to PHMC compliance standards as they appear in the PHMC Code of Conduct, Whistle Blowers, and Conflict of Interest Policies.

· Comply with HRSA, State, local, The Joint Commission, and other accreditation and regulatory agencies standards.

SKILLS:

· Exceptional leadership skills.

· Exceptional ability to communicate with patients, health care staff, public entities insurance, community partners.

· Exceptional ability to develop trainings and provide interdisciplinary education to staff and community partners.

· Strong ability to work with staff across disciplines.

· Strong data analysis and presentation skills.

· Sound judgement and initiative.

· Proficiency with Microsoft Word, Power Point and Excel.

· Comfort and flexibility working in an independent as well as team-based environment.

EXPERIENCE

· Strong experience in managing teams and initiatives.

· Demonstrated knowledge of the patient-centered medical home (PCMH) model and principles.

· Familiarity working as a member of an interdisciplinary team.

· Experience navigating complex healthcare and social services systems.

· Experience working with vulnerable populations substance use and/or mental health disorders for at least two years.

· Certification as Community Health Worker strongly preferred.

EDUCATION REQUIREMENT:

· High School Diploma/GED required.

· A Bachelors’ degree may be substituted for one year of care coordination or community health experience.

PHYSICAL DEMANDS:

· Travel is occasionally required by personal automobile or public transportation.

· Walking from public transportation is required.

· Stair climbing may be required.

· Position may additionally require standing, walking, sitting, use of hands, reach with hands and arms, and talking and listening in-person and virtually.

WORK ENVIRONMENT:

· Moderate noise (examples: business office with computers and printers, light traffic).

· Shared work environment.

· Primary work environment is a medical facility, with a degree of exposure to tuberculosis and other air-borne disease that may require the use of personal protective equipment.
