Basic Qualifications
Education: A high school diploma, GED diploma, or active registry status on the Pennsylvania nurse aide registry; CPR certification or ability to attend and pass course; current certificate from DPW approved medication administration course, depending on shift and unit, or ability to attend and pass course.
Skills: Moderate reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communicative skills; ability to sit, stand, lift, bend, reach, push, and pull; current C.P.R. certification (or eligibility and pursuit of certification); satisfactory completion of pre-employment physical (post-offer). Show a general interest in geriatric care.
Experience: Prior experience preferred, but not required. Minimum one (1) year experience in related positions normally required.
Provides individualized, personal care to residents.
1. Assists with orienting residents and their families to Meadow Glen upon admission and to the facility when transfers occur.
2. Must be able to use principles of good body mechanics in lifting, moving, and transporting residents.
3. Communicates and interacts effectively with the geriatric resident.
4. Performs all job responsibilities in a manner which maintains the resident's rights.
5. Provides individualized care to residents according to the resident's care plan such as bathing, dressing, grooming, etc.
6. Attends to the individual needs of residents while maintaining their comfort and dignity.
7. Assists in maintaining a safe, clutter free and clean environment.
8. Observes for changes in the resident's condition or behavior and
reports these to the LPN shift supervisor immediately.
9. Provides care that maintains the residents’ skin integrity such as changing incontinent residents, turning and repositioning residents in bed etc.
10. Observes for changes in skin integrity and reports these to the LPN shift supervisor immediately.
11. Answers resident's call bells in a timely manner.
12. Follow instructions given by the Resident Care Supervisor, LPN and Administrator.
13. Prepares residents for appointments; such as, beauty shop, activities, doctor appointments, family visits/outings, church services etc.
14. Documents daily
15. Completes required resident assessments and documents accordingly
16. Invite, encourage and escort residents to meals and to activities
17. Demonstrates the ability to meet established behavioral expectations outlined in the Performance Coaching process.
18. Attends all mandatory in-service training programs. All RCAs are required per Department of Human Services regulations to attend at minimum 12 hours of in-servicing as well as an addition 6 hours in-servicing for dementia care.
20. Participates in task force projects as invited.
21. Maintains certification and meets all requirements as a med tech and reports any errors or needs to the nursing supervisor or administrator immediately. Minimizes errors or asks for additional training as needed in meeting med tech requirements.
22. Employees will be designated to attend the med tech training, based on the current needs of the Personal Care Facility. This will be at the discretion of the Supervisor and Administer.
23. Completes annual compliance and HIPAA training and exhibit behavior as set
forth by the Phoebe Ministries Code of Ethics and Business Integrity in the
performance of their duties.

24. Preforms tasks which are supportive in nature to the essential functions of the position, but may be altered or redesigned depending upon individual circumstances.
