Summary:
The HIIM Data Integrity Analyst –Validation is responsible for ensuring the medical record and forms are properly identified and indexed correctly within the established application. The Analyst will identify and validate all required documentation and if requested, verify that all documents have been authenticated before the record is filed as complete. The HIIM Data Integrity Analyst will perform re-analysis, to ensure that all documents have been assigned appropriately and are accurate.

The HIIM Data Integrity Analyst –Validation is responsible for ensuring the medical record and forms are properly identified and indexed correctly within the established application. The Analyst will identify and validate all required documentation and if requested, verify that all documents have been authenticated before the record is filed as complete. The HIIM Data Integrity Analyst will perform re-analysis, to ensure that all documents have been assigned appropriately and are accurate.
Essential Duties and Responsibilities:
- Indexes all scanned documentation within the medical record to the appropriate facility, patient, and document type
- Reviews all scanned documentation for image quality, including but not limited to, page orientation, coloring, and legibility
- Identifies documents needing to be re-scanned due to poor image quality
- Identifies missing pages within the chart that may require additional follow up
- Adheres to established CHS standards/policies and system workflow guidelines to add and re-assign accounts to appropriate work queues for processing and/or follow up.
- Verifies correct scanning of the MRN, account number, and/or document type.
- Verifies quality and correctness of all scanned documents from the imaging vendor and the facility
- Ensures the integrity of the legal medical record
- Performs other duties as assigned
- Communicates daily with the facility HIMD on the validation status
- Communicates via established protocols to local HIM director of missing documents and other needs requiring local facility intervention
- Addresses appropriate questions related to validation completion from the HIIM Data Integrity Quality Manager, HIIM Supervisors, and/or Facility HIMDs
- Actively performs validation on medical records within 12 hours of availability, excluding established facility exceptions
- Meets or exceeds corporate established Medium productivity standards for validation within 6 months of hire.
- Meets or exceeds corporate quality standard of 98% or greater on all validated medical record with an overall critical error accuracy of 100%
- Assists in cross-training other employees as necessary.
- Able to work independently as a remote employee.
- Demonstrates competency of all validated assignments within 60 days of hire
- Requires that employee is able to work a schedule that includes weekend coverage on Saturday and/or Sunday.
- Requires that employee sustain a secure home working environment
- Requires that employee sustain a secure home internet service and router
- Requires that employee ensure company issued equipment stays maintained and intact
Required Experience: Three (3) years’ experience in medical record analysis required
Preferred Experience:
- Previous experience in an HIM Department or office environment strongly preferred
- Experience with electronic medical record imaging systems - Cerner Millennium Advanced Capture
- Analytical skills with the ability to problem solve and identify potential workflow barriers/issues
- Must be able to work independently as a remote employee and prioritize multiple tasks
- Ability to decipher different handwriting types
- Possess strong written and verbal communication skills
- To perform this job successfully, an individual should have knowledge of Cerner imaging software, Google Suite, Microsoft Office applications; Database software, Audio/Video Conferencing.
- Applicant must demonstrate proficiency in use of keyboarding and use of a mouse.
- The Employee is required to read, review, prepare and analyze written data and figures, using a PC or similar, and should possess visual acuity.
- The Employee may be required to have times of prolonged sitting, some bending, stooping, and stretching. In addition, eye-hand coordination and sufficient manual dexterity to operate a keyboard, telephone, calculator, and other office equipment are necessary.
- The Employee is not substantially exposed to adverse environmental conditions and; therefore, job functions are typically performed under conditions such as those found within general office or administrative work.
