JOB SUMMARY

Assist Pinal County residents with access to Community Health Nutrition Programs (CHNP) including the Women Infants & Children (WIC) program, nutritional/breastfeeding counseling, education and training, determine client eligibility for program services, assess clients’ nutrition, breastfeeding and health-related needs, and provide services according to Public Health Services Department (PHSD) policies and procedures under basic supervision.

Example of Duties

TYPICAL CLASSIFICATION ESSENTIAL DUTIES:

  • Assist PHSD clients with information and services for a variety of Community Health Nutrition Programs (CHNP) including the Federal Women Infants & Children (WIC) program, Commodity Supplemental Food Programs (CSFP), and other nutritional/breastfeeding counseling programs and support services and perform duties within scope of authority and training and in compliance with County policies.
  • Interview clients, document income status and current living conditions, and determine eligibility for CHNP services.
  • Evaluate clients’ nutrition/breastfeeding and health-related needs, discuss and document clients’ eating habits, health history, and current medical condition, and work with client to develop personal goals and a nutrition/breastfeeding care plan.
  • Counsel and educate clients on proper nutrition/breastfeeding practices, provide educational material and personalized information on CHNP and PHSD programs, and refer clients to other service providers as needed.
  • Measure height/weight and hemoglobin (blood) testing.
  • Assist clients to access other health care professionals, regional agencies, and community services programs.
  • Maintain case file records and collect, compile, and report data on program participation.
  • Assist with community outreach programs and special projects as assigned.
  • Train and cross-train in a variety of nutrition, breastfeeding and healthcare programs.
  • Maintain absolute confidentiality of work-related issues, customer records, and restricted County information.
  • Perform other related duties as required.

Minimum Requirements

MINIMUM REQUIREMENTS TO PERFORM WORK:

  • High school diploma or GED equivalent.
  • Or an equivalent combination of relevant education and/or experience may substitute for the minimum requirements.
Preferred Qualifications:
  • One (1) year of experience in a medical or social services program.
  • Bi-lingual English-Spanish language skills.
  • Successful completion of training programs by Arizona Department of Health Services.
  • One (1) year of experience in a medical or social services program.

Supplemental Information

Knowledge, Skills and Abilities:

  • Knowledge of Federal and state nutrition/breastfeeding program regulations and requirements.
  • Knowledge of methods, practices, and procedures for assisting public health clients.
  • Knowledge of regional community service agencies, community resources, public health programs, and other public assistance programs for appropriate referrals.
  • Knowledge of principles of record keeping and records management.
  • Ability to explain state laws and County policies and procedures governing public health functions.
  • Ability to use courtesy and respect in providing client services.
  • Ability to operate a personal computer utilizing standard and specialized software.
  • Ability to provide effective client services including interviewing, teaching, counseling, and records management.
PHYSICAL DEMANDS:The work is medium and requires exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects. The work also requires the ability to crouch, feel, finger, grasp, handle, reach, perform repetitive motion, kneel, lift, pull, push, to operate motor vehicles, hear, speak, stand, stoop, walk, and demonstrate mental and visual acuity.
