We are 39 Bed in-house facility where our staff is dedicated to providing comprehensive skilled care, being caring and compassionate, and creating an intentional home-like experience for your loved one. This skilled nursing home in Portland, ME, has licensed, skilled nurses on staff 24/7. Essential Duties and Responsibilities:

  • Administer treatment to residents and patients with temporary or long term disabilities to relieve pain,restore or improve function, and promote healing
  • Assure all treatment is delivered in accordance with an established plan of care
  • Provide clinical support and instruct patients, families, and caregivers
  • Monitor patient response to treatment intervention
  • Complete required forms and documentation in accordance with company policy and state/federal regulations
  • Consulting with patients to learn about their physical condition.
  • Assessing and interpreting patient evaluations and test results.
  • Developing treatment plans using a variety of treatment techniques.
  • Creating personalized fitness-oriented health care programs for patients.
  • Administering medically prescribed physical therapy treatments to relieve pain and improve mobility.
  • Advising patients on exercise techniques.
  • Advising patients and their families about in-home treatment options.
  • Providing educational information about injury prevention, ergonomics and ways to promote physical health.
  • Consulting and collaborating with other healthcare professionals.
  • Documenting patient care history.
  • Complying with rules, regulations, and procedures.

POSITION SUMMARY

Primarily responsible for planning, organizing, developing and directing Physical therapy services in accordance with current applicable federal and state standards, guidelines and regulations. To ensure the highest degree of quality resident care can be maintained at all times.

ESSENTIAL FUNCTIONS

Plan, develop, organize, implement, evaluate and direct physical therapy services as well as its programs and activities in accordance with current regulations and guidelines that govern the skilled nursing facility.

Assumes responsibility for the assessment of patients appropriate to the age of the patient

Assumes responsibility for the care of patients appropriate to the age of the patient

Maintains and completes accurate documentation, department records and reports.

Ensures effective communication regarding PT programs.

Contributes to the daily management of patient care and rehabilitation personnel.

Maintains commitment to professional growth and competency.

Operates and maintains equipment and environment in a safe and orderly manner.

Contributes to the philosophy of quality care and human dignity at the facility.

Contributes to the effective operation of the facility by demonstrating dependability in job performance.

Maintains an efficiency standard of 90% weekly to meet expected facility benchmarks.

Participates with discharge planning, development and implementation of resident care plans.

Assumes the responsibility, authority and accountability of directing the physical therapy assistant.

Performs evaluations/screens in a timely and efficient manner.

Completed documentation in a timely manner in accordance with state and federal regulations.

Maintains appropriate continuing education credits in accordance with state regulations.

Maintains a positive work environment for residents and coworkers.

Ensures proper HIPPA guidelines are followed and maintained.

Education:

Graduate of an accredited school of Physical Therapy

Experience:

Licensed Physical Therapist in the state assigned.

Long term care experience preferred.

Graduates accepted.
