Your Role as a Pinnacle Home Health RN:
As a Home Health Nurse at Pinnacle, you'll make a direct impact on the lives of our patients from the comfort of their homes. Your key responsibilities will include:
Comprehensive Patient Assessment: Conduct initial assessments to determine patients' home care needs, recognizing changes and trends in patient conditions, and adjust care plans as necessary.
Collaborative Care Planning: Develop individualized care plans in conjunction with patients, their families, and healthcare providers, maintaining continuous communication to ensure treatment goals are being met.
Medication Management: Administer medications, monitor patient responses, and instruct patients and their families on proper medication management.
Health Education: Provide patients and their families with education and support regarding disease processes, self-care techniques, and prevention strategies.
Coordinative Role: Act as a case manager, coordinating patient care, facilitating seamless communication between all parties involved, and ensuring that care is comprehensive, continuous, and patient-centered.
Qualifications:
Associate’s or Bachelor’s degree with 1-2 years of recent acute care experience in an institutional setting.
2 years of home health experience preferred.
Active RN license in the state of Florida.
Current CPR certification.
Reliable transportation and valid driver's license.
Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
