Placements USA LLC is Hiring a Nurse Practitioner - Addiction and Family Medicine Near Fitchburg, MA

Under the general direction of the supervising physician, the Medical Director and Health Center, the Advanced Practice Nurse will lead and provide care in our office-based Medication Assisted Treatment (MAT) clinic. This position provides prevention, screening, intervention and treatment related substance use disorders.
Essential Duties and Major Responsibilities:
  • Provides direct patient care for MAT, substance use and other psychiatric disorders both in-person and via telemedicine
  • Works closely with other team members to ensure seamless integration of medical services with whole-person support services
  • Contributes to a unique and attractive culture that values mission-orientation, proactive initiative, ethical behavior, and collaboration
  • Participates in continuous quality improvement (CQI) initiatives
  • Assesses patient's physical and psychosocial health/illness status through history taking and physical examination.
  • Practices within the collaborative model with physicians, nurse practitioners and clinical psychologists
  • Diagnoses, manages, and appropriately refers patients with acute, episodic, or chronic illness according to department protocols.
  • Orders appropriate laboratory and diagnostic tests according to established guidelines.
  • Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
  • As appropriate to the position, participates in specified health promotion, education and/or prevention programs.
  • Ability to observe, assess, and record symptoms, reactions, and progress.
  • Knowledge of legal and ethical standards for the delivery of medical care.
  • Knowledge of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosages and instruct patients in correct usage.
  • Ability to maintain quality, safety, and/or infection control standards.
  • Knowledge of community medical diagnostic and patient care services in area of medical expertise.
  • Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in area of expertise
  • Facilitates and coordinates patient care referrals to specialists and outside agencies
  • Develops and provides educational materials and/or programs to patients, families, and other health care professionals at the Center.
  • Assumes formal teaching responsibilities within area of expertise and precepts nurse practitioner students, medical students, and residents as appropriate.
  • May participate in medical and/or nursing research.
  • Participates in defining nursing standards, practices, and educational needs of patient care staff
  • May share call responsibilities of the service.
  • Ability to maintain quality, safety, and/or infection control standards.
  • Knowledge of community medical diagnostic and patient care services in area of medical expertise.
  • Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in area of expertise.
  • Actively participates in the departmental quality assurance program.
  • Serves on appropriate department and/or hospital committees.
  • Performs other related duties as requires and as assigned.
  • Managing patient care is a team effort that involves clinical and non-clinical staff interacting with patients.
  • Demonstrate understanding and commitment of the health center mission.
  • Demonstrate understanding and commitment to the established Values and Standards.
Minimum Qualifications:
  • Massachusetts Licensed Registered Nurse required
  • Certified in Massachusetts to practice in the expanded role as a Family Nurse Practitioner required
  • Current active, unencumbered DATA 2000 Waiver to allow for prescribing MAT
  • CARN-AP preferred

  • Ability to work across teams and functions, self-starter who quickly identifies what needs to be done and takes ownership
  • Certified in CPR/AED or equivalent.
  • Demonstrated mastery of a specialty area or other related field normally acquired by 3-5 years progressively responsible nursing experience preferred.
