Care Coordinator
Plains Area Mental Health, Inc. is a Community Mental Health Center with 10 locations in Northwest Iowa. PAMHC is seeking a full-time Care Coordinator for our Cherokee, Le Mars, or Storm Lake location. Travel is required. This person will provide services to Integrated Health Services members. IHS is a model of healthcare delivery that utilizes a patient-centered holistic approach to coordination of care for the Medicaid eligible mental health population. Position is 40 hours per week Monday through Friday with occasional evening work.
Pay Range:
$21.64 to $23.80 depending on experience

Position Summary:
Primarily responsible for understanding and monitoring member’s concerns and whole health resources needed, making and tracking referrals, follow-up on referrals, completing assessments, and coordinating the care provided by behavioral, medical, and social service providers. Assist in the Assessment and Care Plan development and assist members in meeting their goals.
Requirements:
Bachelor's Degree in Social Work or Human Services Field
Valid Driver’s License and Insurance
Preferred Qualifications:
Bilingual a plus, but not required
