General Summary
Perform timely and accurate follow-up with PPSO patients, including but not limited to; Family planning, Vasectomy, Colposcopy, and Abortion patients, in all the PPSO affiliate clinics.
Responsible for reviewing clinical lab results and other diagnostics in Electronic Health Record (EHR) system to identify and follow-up on normal and abnormal results, utilizing PPSO policies and Standing Orders.
Provides direct services to clients using Standing Orders and maintains the efficiencies of the clinic day through their awareness of the clinic schedule, walk-in clients awaiting service, and clients receiving care.
Create and manage patient follow-up care plans per PPSO policies, protocols, and Medical Standards and Guidelines (MS&G's)
Triage patients by utilizing the nursing process and RN assessment skills via telephone.
Support patients with education, providing medical information, and referrals, largely by telephone, FAX, EHR. Coordinate closely with Health Center managers, Call center manager, Clinicians, Follow-up Coordinator, Case Management Team members, and Administration to ensure high-quality care and excellent patient experience.

Essential Duties and Responsibilities
:
  • Coordinate follow-up plans for PPSO patients per Medical Standards and Guidelines (MS&G's) and per protocols.
  • Review and sign off normal/Neg clinical lab results in the EHR. Clinical consult for abnormal results.
  • Notify patients of abnormal test results and provide education of condition.
  • Provide STI treatments, lab orders, and refills per RN Standing Orders.
  • Respond to patients who call or inquire via EHR with questions about results, procedures, referrals, records, and others, as assigned.
  • Actively participates in the delivery of reproductive health services including abortion care. Participation can include, but is not limited to, direct nursing care, referrals, information, education, counseling, dispensing, and screening. Limited sonography and minimal and/or moderate sedation if privileged by PPSO for this.
  • Work with the Clinic Manager, Leadership Team, and the Clinic Medical staff to assure that the clinic offers high quality and medically appropriate family planning services that are sensitive to the needs of our clients.
  • Review and signs charts to confirm compliance with Planned Parenthood protocols and medical standards. (Examples of visits overseen would include pill refills and rechecks, emergency contraception visits, injections, pregnancy tests, hormonal contraceptive visits and STI screens without a physical exam.
  • Make nursing decisions based on RN scope as defined by Oregon Board of Nursing and standing orders of the Medical Director and Planned Parenthood, consulting the Clinician Supervisor, Medical Director, or assigned clinician to Clinical Consult pool when lab values, patient symptoms, or patient concerns are outside the norm as defined by the MS&G’s, PPSO protocols, PPSO procedures, and/or PPSO Standardized Procedures.
  • Accept individual and joint responsibility for clinic efforts to comply with all organizational quality assurance guidelines and pertinent government regulations including OSHA, CLIA and HIPAA.
  • Ensure that all drugs are kept in a locked location unless a physician, APRN, or Registered Nurse, is present.
  • Accurate documentation of beginning/end/waste counts for controlled substances and maintaining logs.
  • Counsel patients on the use, risks, benefits, and side effects of any new medications prescribed and ensure a drug information fact sheet is given to the client.
  • Dispense new medications to patients and review and approve refill dispensing using Standing Orders.
  • Assists/performs IV access/medication, injections, immunizations, CLIA waived laboratory tests, RH typing, and phlebotomy.
  • Participate in the training of technical skills as assigned such as phlebotomy and ultrasound.
  • Provide client telephone consultation via Standing Orders.
  • Assure that appropriate client follow-up occurs by scheduling return or referral appointments for clients as needed.
  • Maintain patient medical records in good order utilizing standard language and approved abbreviations and following accepted medical standards for charting and documentation.
  • Review patient records for accuracy/completion in the recording of the visit.
  • In conjunction with the Clinic Manager, assist with training and education of HCA staff if requested.
  • Work cooperatively with the Clinic Manager to assess and evaluate the performance of HCA staff if requested.
  • Participates in team meetings for system improvement, as well as problem identification and resolution. Attend staff and special meetings as scheduled or requested.
  • Adhere to procedures and protocols ensuring timely patient notification including the mailing of appropriate reminder letters.
  • Communicate with Health Center staff via EHR, telephone, TEAMS, ZOOM, and email as necessary.
  • Consult with Clinician Supervisor, Director of Clinical Services, or Medical Director to confirm clinical accuracy and appropriateness of care plans/follow-up as needed.
  • Maintain ongoing communications with clinicians and other health center staff to ensure continuity of patient care.
  • Follow alternate procedures to maintain Follow-up system in event of EHR downtime.
  • Complete RQM audits, if assigned, under direction of the Clinician Supervisor or CQI coordinator.
  • In EHR system, regularly run and monitor lab reports for pending labs with next action dates (NAD).
  • Monitor and manage database of unassigned/unmatched labs to ensure appropriate follow-up.
  • Referral follow-up:
Follow-up on internal and external patient referrals with accurate documentation. Ensure response to requests occurs in a timely manner.
  • Assist in the maintenance of the referral manual per PPSO Medical protocols.
  • Patient Advice Line and Triage:
Provide first-line telephone support to current PPSO clients by providing reproductive health care information and telephone services in an effective, efficient, and nonjudgmental manner.
Educate patients regarding issues related to sexuality, STl's, birth control, pregnancy-options, reproductive health issues, abortion (surgical and medication) and vasectomy. Refer clients to written literature and other information sources upon request.
  • Communicate openly with the clinic manager and supervisor about the status of the clinic.
  • In cooperation with Leadership, develop recommendations for improving patient care and providing quality customer service.
  • Demonstrate leadership in clinical skills with staff by maintaining professional and technical knowledge.
  • Assist in the development of systems to streamline patient follow-up processes and procedures.
  • Assist as needed in developing and updating the list of regional contact persons from outside agencies for patient referrals.

Qualifications
:

Minimum Education and Experience:
Position requires a valid Oregon States Nursing license, and current BLS card. Experience in Community Health, Reproductive Health, or Women's Health desirable.
ACLS is required if privileged in Moderate sedation and can obtain at that time.

Preferred Education and Experience:
Previous Planned Parenthood experience. Two years as a full scope RN is preferred. Experience in supervision, instruction, and evaluation desirable.
