POSITION TITLE: Transgender Care Follow-Up Specialist
STATUS: Full Time, Non Exempt
REPORTS TO: Program Director, Gender Affirming Hormone Therapy
SALARY: $27,00-$35.00/HR

POSITION SUMMARY
The Transgender Care Follow-Up Specialist will work in the case management and clinical follow-up of our transgender and gender expansive patients. They will be a core member of the Transforming Community Transforming Care (TC2) team. Primary responsibilities include completing prior authorizations for patient’s prescriptions, managing pharmacy questions, answering patient questions, coordinating letters of support, coordination of lab tests to be done at outside labs, coordination with patient navigators and other TC2 team members, and collaboration with clinical providers for ongoing clinical care. Must be able to work with minimal supervision, be organized, with the ability to multi-task and provide timely communication with colleagues and health center staff.
PRIMARY DUTIES PERFORMED
·Answer patient calls, texts or electronic messages regarding clinical medical questions in collaboration with the Program Director of Gender-Affirming Hormone Therapy. Triage phone calls, work with patient navigators to address patient needs
·With patient navigators perform initial assessment and introduction to the program, including reviewing financial needs and available resources.
·Responsible for managing gender expansive patient follow-up plans in collaboration with responsible clinician.
·Participate in TC2 team meetings.
·With Patient Navigators, responsible for managing communication from patients on the Patient Portal.
·Responsible for managing tasks assigned by providers.
·Review medical records from outside facilities and task to the appropriate provider. Should be familiar with Release of Information (ROI) forms, how to complete correctly and send.
·Complete medication prior authorizations as needed.
·Make appropriate appointments in EPM for patients for treatment and follow-up of abnormal lab results, in-house referrals, and urgent lab follow up.
·Monitor voicemail, return appropriate phone calls, and document client information and forward calls as appropriate.
·Adhere to affiliate policies on professionalism, wait time on the phone, and the system for addressing client complaints.
·Provide regular feedback to the Program Director regarding training and continuing education needs.
·Participate in the affiliate efforts to achieve established productivity and revenue cycle goals.
·Participate in affiliate efforts to improve the patient experience.
·Perform other duties as required.
·Periodic travel to affiliate locations may be required
Clinical:

·Document appropriately and thoroughly in EHR
·Lab training and duties as needed
·Patient education
·Participate in the affiliate efforts to achieve established productivity and revenue cycle goals.
QUALIFICATIONS
·Ability to work with people of diverse backgrounds and lived experiences.
·Knowledge of or lived experience with gender affirming hormone care.
·Experience working in a healthcare setting including accurate assessment of vital signs, ability to give IM or SQ injections and phlebotomy.
·Excellent verbal and documentation skills. Ability to manage remote internet connection and devices.
·Ability to work effectively and cooperatively with clinicians and other professional and volunteer members of staff
·Ability to demonstrate sensitivity and empathy to patients and family members
·Knowledge of quality assurance and risk management standards
·Bilingual skills are a plus.
·For the right candidate position can be remote-hybrid. Will be based in Springfield, MO Health Center.
