Our client is hiring multiple Mental Health Specialists and Case Managers to support their programs throughout Modesto. As a Mental Health Specialist/Case Manager with one of our programs, you'll be part of a company that prioritizes employee well-being while providing essential mental health and housing support services to adults struggling with severe mental illness.

You'll be working either in our lower-acuity programs serving low-income clients with severe mental illness. Day-to-day you'll be meeting your clients at our facilities and meeting with individuals in the field (home, school, workplace, etc.) who are not able to get to our facilities.

Day-to-day you'll be engaged in peer counseling, crisis intervention, leading support groups and teaching coping skills, and providing case management services to clients struggling with mental illness and seeking help. Most clients are low-income and facing housing or employment insecurity as a result of their SMI.

What You'll Do:

  • Help clients achieve their goals and navigate community life.
  • Offer support in medication management, housing, counseling, and advocacy.
  • Lead and guide clients in the absence of the Program Director.
  • Advocate for clients across all aspects of treatment and service access.
  • Support individualized service plans for each client.
  • Keep accurate client records and complete necessary documentation.
  • Meet clients regularly at various locations.
  • Collaborate with clients' families and caregivers with consent.
  • Coach clients in social skills, money management, personal hygiene, and job readiness.
  • Assist clients in accessing benefits like Medi-Cal.
  • Develop financial plans for clients needing Representative Payee Services.
  • Work within a team to provide comprehensive case management.
  • Provide counseling consistent with program philosophy.
  • Drive for program-related tasks using personal vehicle.
  • Ensure client safety and wellbeing at all times.
  • Complete paperwork promptly and efficiently.

Requirements:

  • 2 years of mental health experience (with a Bachelor's) or 4 years of mental health experience (without a Bachelor's).
  • Able to work a standard, Monday-Friday 8:30am-5pm schedule.
  • Reliable transportation, valid CA driver's license, and clean driving record.
  • Able and willing to work onsite full-time (no remote work).
  • Able and willing to meet clients in the field (at their homes, schools, workplaces, etc.) and occasionally transport them to appointments in your vehicle.
