Plymouth Harbor on Sarasota Bay is Hiring a Flex LPN - Assisted Living Near Sarasota, FL

The Licensed Practical Nurse (LPN) is responsible for providing direct care to the residents, and providing supervision of the day-to-day activities of the nursing assistant staff in accordance with current federal and state standards, guidelines, and regulations that govern the Assisted Living including Plymouth Harbor policies and procedures.
DUTIES/RESPONSIBILITIES
1. Ensure that documentation of residents’ daily medical record is accurate, comprehensive and timely.
2. Receive, document, and implement physicians’ orders in an accurate and timely manner.
3. Coordinate admit and discharge of residents to and from the ALF.
4. Ensure timely response to resident requests/call lights.
5. Ensure the proper recording and accurate count of schedule drugs at all times.
6. Ensure security of locked ALF areas; monitor that only authorized staff is in possession of ALF keys.
7. Ensure accurate and comprehensive reporting of resident condition via shift to shift reporting.
8. Interpret departmental policies and procedures to personnel, residents, visitors, and family members.
9. Visit each resident at least once during a shift to ensure appropriate care is being rendered and act accordingly.
10. Ensure that Residents’ Bill of Rights is followed at all times.
11. Ensure that residents have a clean, safe, orderly, and comfortable environment.
12. Ensure that departmental personnel, residents, and visitors follow established policies and procedures at all times.
13. Assist in identifying departmental problems and subsequent resolutions.
14. Make recommendations to the Director of Nursing regarding staffing, improvement of services, and potential problem areas.
15. Perform administrative duties as required by departmental policies and procedures and report to the Director of Nursing.
16. Assist in coordinating transportation to other health care facilities and individuals as indicated.
17. Coordinate health services, i.e., admissions, discharges, etc.
18. Ensure that a level of medications, medical supplies, equipment, and other supplies is maintained.
19. Complete medical forms according to departmental policies and procedures.
20. Notify the resident’s attending/alternate physician, family, or guardian, of any change in condition, accident or incident according to established facility policy.
21. In cases of death, implement arrangements according to resident’s, family’s, or guardian’s wishes and in compliance with facility policy.
22. Evaluate emergencies, determine emergency measures to be used, and notify the Director of Nursing of the emergency.
23. Collect laboratory specimens as ordered.
24. Perform prescribed monitoring and diagnostic and therapeutic procedures.
25. Assist medical professionals, as needed, during on-site visits.
26. Follow Universal Precautions and Infection Control procedures. Adhere to isolation techniques when appropriate.
27. Comply with Florida State statutes regarding required continuing education credits.
28. Attend and participate in workshops, seminars, and mandatory in-services as approved or required.
29. Maintain work area, equipment and supplies in a clean, sanitary and organized manner. Ensure that work areas are neat and confidential materials are properly stored before leaving area on breaks, end of work day, etc.
30. Work safely; complying at all times with Plymouth Harbor safety standards.
31. Call 555 in the event of an emergency.
32. Discharge properly such other responsibilities and duties as the Director of Nursing, Assisted Living, may direct.
SUPERVISORY RESPONSIBILITY
  • The LPN has supervisory responsibility for nursing assistants.
QUALIFICATIONS
Education:
  • Graduate of an accredited school of licensed practical nursing.
Experience:
  • Minimum one year in the field of Long-Term Care preferred.
General:
  • Must have good organizational skills.
  • Must be a detail-oriented person and able to perform duties with great accuracy on a daily basis.
  • Must have excellent personal skills, demonstrating the ability to conduct oneself in a non-controversial style; a style that inspires the respect of others and promotes a sense of trust in the individual’s competence.
  • Must have good communication skills; speaking, writing, and listening.
  • Must be fluent in English.
  • Must have good computer skills with the ability to learn and use proprietary software as required.
CERTIFICATES/LICENSES/REGISTRATIONS
  • Must have valid certification as a Licensed Practical Nurse in the State of Florida.
  • Must have a current CPR (Cardiopulmonary Resuscitation) certification.
