The Behavioral Health Care Manager (BHCM) will join a dedicated multidisciplinary team to provide comprehensive support to resident populations with the goal of lessening behavioral health inequities and improving wellbeing. The BHCM plays a critical role in the coordination of needed services, including effective, collaborative communication with residents, the internal clinical team, external providers, and the Permanent Supportive Housing (PSH) team, using a person-centered and harm reduction approach.

Plymouth Housing is a nonprofit organization that develops and operates permanent supportive housing for single adults who have experienced chronic homelessness. With apartment buildings throughout King County, Plymouth is helping the most vulnerable members of our community leave homelessness behind forever. At Plymouth, we’re not simply trying to get people off the streets. We provide supportive services like case management and health care so that our residents can thrive long-term. As part of our strategic vision, we are building internal behavioral health services and transforming our systems to address rapid growth and the changing needs of residents.

WHAT YOU CAN EXPECT TO BE DOING IN THIS ROLE AT PLYMOUTH HOUSING

Resident Support

  • Collaborate with residents, PSH Case Managers, and team members to assess behavioral health needs and develop care plans.
  • Facilitate access to behavioral health, substance use, and medical treatments.
  • Connect residents with internal and external clinical resources.
  • Administer and follow-up on symptom screenings.
  • Conduct ongoing assessments to monitor residents' health status and treatment progress.
  • Provide support and advocacy for residents navigating the healthcare system.
  • Maintain accurate documentation of interactions and interventions.

Team Collaboration

  • Partner with PSH Case Managers to overcome barriers to care.
  • Participate in team meetings, providing input on resident care plans.
  • Address Social Determinants of Health (SDoH) and integrate clinical insights into care.
  • Inform clinicians and providers when treatment goals are unmet and follow up with residents.
  • Support residents in setting and achieving their goals.

QUALIFICATIONS

  • Master’s degree from an accredited university or college. Master’s in social work is preferred.
  • Active associate-level clinical licensure in Washington State in one or more of the following:
    • Licensed Family Counselor;
    • Marriage and Family Therapist Associate License;
    • Mental Health Counselor Associate License.
  • Experience working with individuals experiencing behavioral health challenges, substance use disorders, and homelessness.
  • Knowledge of trauma-informed care, harm reduction, and evidence-based practices in behavioral health.
  • Strong communication and collaboration skills.
  • Ability to work effectively in a multidisciplinary team environment.
  • Commitment to cultural humility, equity, and social justice.
  • Experience in permanent supportive housing or related programs is preferred.
  • Familiarity with local resources and community networks is a plus.
