The Lead Behavioral Health Case Manager (LBHCM) will join a dedicated multidisciplinary team to provide comprehensive support to resident populations, with the goal of lessening behavioral health inequities and improving wellbeing. The LBHCM plays a critical role in coordinating needed services and overseeing the Behavioral Health Case Managers (BHCMs), ensuring effective, collaborative communication with residents, the internal clinical team, external providers, and the Permanent Supportive Housing (PSH) team. The position emphasizes a person-centered and harm reduction approach.

Plymouth Housing is a nonprofit organization that develops and operates permanent supportive housing for single adults who have experienced chronic homelessness. With apartment buildings throughout King County, Plymouth is helping the most vulnerable members of our community leave homelessness behind forever. At Plymouth, we're not simply trying to get people off the streets. We provide supportive services like case management and health care so that our residents can thrive long-term. As part of our strategic vision, we are building internal behavioral health services and transforming our systems to address rapid growth and the changing needs of residents.

WHAT YOU CAN EXPECT TO BE DOING IN THIS ROLE AT PLYMOUTH HOUSING

  • Mentor, coach, support, and guide BHCMs.
  • Assist with onboarding, training, and case management techniques.
  • Serve as the first point of contact for BHCM queries.
  • Assist the Residential Services Manager (RSM) in daily operations of the behavioral health team.
  • Ensure service continuity and consistency in policy and procedure implementation.
  • Lead peer learning sessions for quality service delivery and compliance.
  • Facilitate communication with RSM and other departments for partner relationship alignment.
  • Collaborate with residents and team members to assess behavioral health needs.
  • Develop individualized care plans and facilitate access to treatment.
  • Connect residents with clinical resources.
  • Administer symptom screeners and follow-up assessments.
  • Conduct ongoing assessments of residents' behavioral health status.
  • Partner with PSH Case Managers to identify barriers to care.
  • Provide support and advocacy for residents in the healthcare system.
  • Use BH interventions to de-escalate situations.
  • Maintain accurate and timely documentation of resident interactions.
  • Support residents in setting and attaining their goals.

SOME KNOWLEDGE, SKILLS, AND ATTRIBUTES WE NEED FROM YOU

  • Comprehensive knowledge of behavioral health and substance use disorders.
  • Understanding evidence-based treatment modalities and trauma-informed principles.
  • Familiarity with harm reduction strategies and philosophy.

  • Knowledge of crisis intervention techniques.
  • Familiarity with local behavioral health resources, social services, and community networks.
  • Strong verbal and written communication skills for diverse interactions.

  • Proficiency in conducting assessments, developing care plans, and coordinating services.
  • Strong advocacy skills to support residents in healthcare navigation.

QUALIFICATIONS

  • Master's degree from an accredited university or college. Master's in social work is preferred.
  • Active independent-level clinical licensure in Washington State in one or more of the following:
    • Licensed Independent Clinical Social Worker (LICSW);
    • Licensed Mental Health Counselor (LMHC);
    • Licensed Marriage and Family Therapist (LMFT).
  • Minimum of three (3) years of experience working with individuals experiencing behavioral health challenges, substance use disorders, and homelessness.
  • Minimum of two (2) years working as a case manager, with at least one (1) year of experience in a leadership or supervisory role.
  • Valid Washington State Driver License and insurable driving record.
  • Experience in permanent supportive housing or related programs is preferred.
  • Motivational interviewing or Interactive Screening Program (ISP) is a plus.
  • Familiarity with local resources and community networks is a plus.
