Location: San Mateo, CA Summary: The Hospice Scheduler is crucial for coordinating staffing across multiple hospice locations to ensure safe and effective care delivery. Responsibilities include scheduling Case Managers, Team RNs, Hospice Aides, Medical Social Workers, and Chaplains, maintaining weekend and holiday schedules, and ensuring timely communication between patients, caregivers, and staff. The role also involves facilitating interdepartmental communication and cooperation. Job Accountabilities
Ensure continuity of care by scheduling appropriate staffing
Maintain team assignments and coordinate visit schedules
Communicate patient status changes and coordinate admissions
Monitor and adjust schedules, including on-call and weekend assignments
Assist with training and PTO coordination
Generate and manage reports, maintain documentation standards
Participate in quality improvement projects
Manage supplies and resources, control overtime costs
Qualifications
Education: HS Diploma required, Associate’s in a related field preferred
Experience: 2 years in home care scheduling or medical staffing
Skills: Proficient in medical terminology, computer skills, Microsoft Word and Excel, excellent communication and organizational abilities, ability to prioritize and work under minimal supervision
Must be detail-oriented and capable of working in a demanding environment
Developed clerical and computer skills
Ability to handle urgent staffing needs and communicate effectively
Proficiency in managing schedules and documentation
Ability to maintain professional relationships and respond to emergencies
