Clinical Pharmacist – Pain Management 

NH Jacksonville, FL

Position Duties and Responsibilities:

  • Work closely with clinical staff in developing patient wellness programs such as pain management, behavioral health, sleep hygiene, and smoking cessation programs. 
  • Facilitate prescription monitoring reports. 
  • Utilize pharmacy electronic health record (EHR) computer and robotic systems for dispensing medications, document patient care and review patient care rendered by medical providers in other clinics or MTFs in accordance with MTF protocols. 
  • Develop knowledge and understanding of the pain stepped-care model, Tricare Uniform Formulary Tool, formulary and nonformulary medication screening, pain clinical interventions, appropriateness of therapy, workflow operations, and polices. Participates, as non-voting member, in customer services initiatives, performance and quality improvement measures and activities designed to enhance pharmacy services and patient safety, medication adverse event reporting and tracking event reporting and tracking, patient consultations, prescription transfers, medication reconciliation, and clinical pharmacy after supplemental privileges have been granted. 
  • Excellent written and oral communication skills to perform the accurate and timeliness entry of records, encounter closures within 72-hours, reports, and documentation, both written and electronic communication of all information, as appropriate, to include pain patient agreements, pharmacotherapeutic treatment findings and recommendations. 
  • Possess knowledge of pharmacokinetic and pharmacogenomic dosing as it pertains to complex medication regimens including, but not limited to, opioids, antidepressants, antipsychotics, and substance use disorder medications. 
  • Participate as a non-voting member on a multidisciplinary team with other health care providers providing consultative services pursuant to optimal health care delivery to patients with complex medication regimens, including to provide drug therapy recommendations, perform polypharmacy management, and provide patient education regarding changes in drug therapy. 
  • Provide, assess, and review information regarding the general administration of medications, safety, efficacy, toxicity, interactions, investigational drugs, and bioavailability. Monitor and review drug therapy for rational indications, dosing, adverse drug reactions, and complications of multiple drug therapies in all patient care settings. 
  • Counsel on self-monitoring techniques to emphasize medication adherence and provide compliance aids, such as medication charts and pill boxes. 
  • Provide pharmacologic and non-pharmacologic therapies to include prescribing in accordance with DHA, DoD, and MTF policies. Auricular acupuncture may be performed as treatment, if the HCW possess the appropriate training and certification per DHA PI 6025.52, and the HCW is granted the appropriate clinical privileges by the MTF Director, as required for comprehensive pain management. Clinical Pharmacist issued supplemental privileges shall develop appropriate treatment plans and monitor results assessing the patients’ outcome to the suggested therapy. 
  • Order consults and referrals, as appropriate. 
  • As appropriate within established procedures, protocols, and scope of practice, provide telehealth-medicine and telehealth-consultations for patients. 
  • Conduct limited physical assessments relevant to the patient’s medications and comorbidities. Examples of this may include. but are not limited to, vital signs and diabetic foot exams. Perform opioid use screenings and questionnaires such as Patient Health Questionnaire-2 (PHQ2), Patient Health Questionnaire-9 (PHQ9), General Anxiety Disorder-7 (GAD7), Opioid Risk Tool (ORT). 
  • Facilitates quality safety measures for pain management including, but not limited, to review of controlled substance use agreements, need for urine drug screens, need for opioid-reversal medication, and high-dose opioid use. 
  • Participate as a non-voting member on Naval Hospital Jacksonville committees and Quality Management Boards including, but not limited to, Long-term Opioid Therapy and Safety (LOTS) Committee, Pharmacy and Therapeutics (P&T) Committee, medication use, nutritional support, The Joint Commission tracers, and utilization review by gathering information and drug use related to quality assurance of patient care. Contractors are not authorized to participate as a voting member of any Board or Committee. 
  • Adhere to the Customer Service Accountability – Contract employees providing services at NHJ and supporting facilities shall adhere to the Customer Service standards specified in DHA and NAVHOSPJAXINST 6320.7H or updated version. 

Minimum Qualifications: 

  • Doctor of Pharmacy (Pharm.D.).
  • Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
  • Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy or documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy.
  • Possess a minimum of 12 months of experience within the preceding 36 months as a Clinical Pharmacist performing direct patient care clinical activities in an ambulatory care pharmacy setting. 
  • Current, full, active, and unrestricted license to practice as a pharmacist.
  • Must be a US Citizen.
