Poydras Home, a CCRC in uptown New Orleans, is looking for a Social Services Coordinator to assist the Admissions Coordinator/Social Worker with attending to the social services needs of our elders and their families.

Qualifications include:

· Holds a Bachelor Degree and preferably a Master’s degree (LMSW/LCSW) in applicable field

· Has experience with the aging and understands the aging process and enjoys working with elders, understands their special needs, and is dedicated to the profession and mission of the organization

· Maintains an active liaison with the community network and keeps abreast of current trends in aging

· Has extensive experience working with Microsoft Word, Outlook, Excel and the Internet

· Has ability to communicate with the residents and families in a professional and therapeutic manner as well as the ability to communicate with staff, the Board of Trustees, and Professionals in an appropriate manner

Job Tasks include:

• Responds to all telephone calls/emails regarding elder needs or concerns
• Provide one-on-one support to family and/or elders
• Attend weekly resident care plan meetings for both Nursing and Assisted Living elders
• Conducts admission notes and weekly staffing notes for nursing elders
• Social History within 7 days of admission
• Quarterly staffing notes and other progress notes as needed
• Attend resident council meetings for all areas of care within Poydras Home

Job Type: Part-time

Pay: From $20.00 per hour

Expected hours: 20 – 25 per week
