CentraCare is hiring an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to the Internal Medicine team in Willmar, Minnesota.
ResponsibilitiesAs an Internal Medicine (IM) Provider, you will be a member of our collaborative team caring for adults ages 18 and up at the Main Clinic in Willmar, MN.In this role, you will manage acute and chronic conditions in the clinic, seeing new patient consults and follow-ups as part of your routine practice.Additionally, nursing home rounds and rotation into Urgent Care are part of the practice in this setting.

Schedule
- 0.8 - 1.0 FTE
- Primarily Monday through Friday day shifts
- In this position you will work in both the clinic and nursing home setting
- Approximately 3 to 4 weekends a year in Urgent Care rotation (Saturday and Sunday comprise a weekend)
- No call, nights or holidays
- Completion of a formal nurse practitioner or physician assistant program approved by a national accrediting agency
- Certified as a Nurse Practitioner or a Physician Assistant
- Current or pending licensure in the State of Minnesota
- BLS Certification
