Prairie Harvest Mental Health is Hiring a Housing Support Specialist Case Manager Near Grand Forks, ND

OVERVIEW OF ORGANIZATION

Prairie Harvest Mental Health provides community and facility-based services to vulnerable adults. Quality of life is greatly enhanced through improved living environment, access to services and programs and the opportunity for employment.

POSITION SUMMARY

The Housing Support Specialist Case Manager is responsible for supporting tenants at facilities to remain housed. This position involves interacting with tenants, guests, and service providers to monitor the activities of the building and provide interventions and support, as needed along with supervision of janitors at the facilities.

This position will engage and support tenants with their needs to aid in the development and generalization of individual skills to improve quality of life and promote independent living in the community, while making changes toward recovery and management.

SPECIFIC DUTIES

  • Provide assistance with basic needs of the tenants.
  • Encourage and assist tenants in connecting within the community and in the community at large through formal or informal activities. This would include but not limited to social, recreational, and cultural.
  • Provide harm reduction support and knowledge.
  • Assist tenants in paying rent, if applicable, and meeting their housing obligations.
  • Support tenants to understand their rights and responsibilities as a leaseholder.
  • Support tenants with communication with their landlord and conflict resolution.
  • Advocate for tenants’ wants and needs.
  • Act as a liaison to additional supportive services and providers.
  • Understand and adhere to program policy and procedures and lease agreements.
  • Support tenants to understand program policy and procedures and lease agreements.
  • Participates in regulation of work and paper flow, including tenant assessments, housing stabilization and service plans to tenants who accept individualized supports.
  • Complete all documentation in accordance with agency standards including documentation in an electronic health record, daily log, and incident report documentation; this includes, but is not limited to, content expectations and timeliness standards.
  • Gives guidance and supervision, either directly or indirectly, to janitorial staff employed at the facilities. This may include responsibility for personnel actions (in consultation with the Human Resource Director) along with scheduling, timecard review and leave time approval.
  • Report to the Director of Clinical Services for guidance and problem solving.
  • Attend and participate in training as directed and as required to maintain the ability to provide services.
  • Attend Prairie Harvest Mental Health staff meetings and other staff meetings as needed per client assigned.
  • Other duties as assigned.

PHYSICAL REQUIREMENTS

  • Ability to stand, walk, bend, stoop, crouch, kneel and sit.
  • Ability to use hands/fingers to handle or feel and reach with hands and arms.
  • Most physical demands require repetitive movement.
  • Occasional lifting and moving up to 35 pounds.

SUPERVISORY RESPONSIBILITIES

  • Supervises janitorial staff at the facilities.
  • Carries out supervisory responsibilities in accordance with the organizations’ policies and applicable laws.
  • Responsibilities may include training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

QUALIFICATIONS – EDUCATION/EXPERIENCE

  • Master's degree in psychology, social work, counseling, education, child development and family science, human services or communication disorders. Or a bachelors' degree in one of the above fields and two years of work experience in the respective discipline. Or a Licensed Baccalaureate Social Worker (LBSW). Official transcripts and degrees are required at the time of employment offer, copies may be submitted with application or interview.
  • Supervision of staff.
  • Training/teaching experience in social and/or recreational skills.
  • Knowledge of the principle concepts needed to perform and/or direct duties related to the care and treatment of the mentally ill.
  • High degree of interpersonal skill to be able to communicate with, motivate and/or facilitate behavioral changes in others.
  • Knowledge of development and dynamics of family and human interactions.
  • Working knowledge of community services that support individuals with disabilities.
  • Ability to communicate clearly and concisely in written and verbal form.
  • Ability to develop new programming and job development.
  • Knowledge of local resources available to consumers in the community.
  • Ability to represent the agency by public speaking at various community functions/service clubs.
  • Computer literacy to include Microsoft Office programs.
  • Knowledge of Medicaid Programs.

TRAINING/CERTIFICATION

  • CPR/First Aid
  • Valid Driver’s License/Insurable Driving Record
  • Defensive Driving
  • Review and comply with competencies for standards of practice in The Substance Abuse and Mental Health Services Administration (SAMHSA) Core Competencies for Integrated Behavioral Health and Primary Care.
