Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency. We currently have opportunities for Direct Care Workers to support Premier’s Respite Innovation Program in the Hyannis, MA areas of Massachusetts.

The Direct Care Worker for the Massachusetts Respite Innovation Program will work directly with program participants and families to provide respite care services in the home setting.

Requirements:

• One to two years working in healthcare, primarily in the home health setting or working with a geriatric population.
• Completed and passed the Massachusetts HHA Exam.
• Completed and passed the Massachusetts CNA certification Exam.
• Must meet transportation, language and cultural requirements specific to the program needs.
• Bilingual Spanish required.

Essential Job Responsibilities:

1. Appropriately assist program participants with personal care activities and provide companionship tasks in accordance with respite needs including:

• Baths (tub, shower, bed).
• Mouth care, denture care, skin care, foot care (no toenail cutting).
• Grooming (shampoo, comb hair, shave, file nails).
• Elimination (toilet, commode, urinal, bedpan).
• Supervision of transfers.
• Dressing and undressing activities.
• Supervision with ambulation.

2. Appropriately assist with household tasks directly related to the participant's health:

• Planning and preparing nutritious meals.
• Light housekeeping of client's immediate environment.
• Laundry of client's bedding and clothing

3. Provide reminders for self-administration of medication.

4. Utilize Premier’s Real Time Data technology to capture participant’s changes and identify declines in health efficiently.

5. Report any changes in the client's condition or incidents immediately.
