Work Schedule:
- Monday to Friday: 9:00 a.m. to 6:00 p.m., with occasional extended hours until 8:00 p.m.
- Participation in weekend community outreach events or health fairs may be required.
- Average weekly commitment of approximately 40 hours.
- Conduct comprehensive health assessments and screenings for individuals and families.
- Diagnose and manage acute and chronic medical conditions, prioritizing preventive care and health promotion.
- Collaborate with local organizations to develop and implement health education programs.
- Provide culturally sensitive care while advocating for patient rights and healthcare accessibility.
- Facilitate patient referrals and ensure coordinated follow-up care for optimal outcomes.
- Maintain accurate and compliant medical records.
- Participate in community outreach events and health fairs to raise awareness and offer health screenings.
- Stay updated on community health trends, evidence-based practices, and relevant policies.
- Valid Nurse Practitioner certification and active state nursing license.
- Master's degree in nursing from an accredited institution.
- 3 years of clinical experience, preferably in community or public health settings.
- Strong interpersonal and communication skills, with the ability to work effectively with diverse communities.
- Commitment to social justice, health equity, and serving vulnerable populations.
- Competitive annual salary: $110,000-$130,000, commensurate with experience and qualifications.
