Presbyterian Communities of South Carolina is Hiring a Social Services Coordinator Near Summerville, SC

As a Social Services Coordinator, you will play a vital role in assisting residents with their adjustment to their new living conditions and community. You will serve as the first point of contact for concerns, initiate resolutions, and facilitate communication between residents and their families. Your responsibilities will include:
  • Assisting residents and patients with various forms of communication to family members.
  • Supporting residents, patients, and their families with future planning.
  • Reviewing Bill of Rights and care plans, ensuring appropriate signatures are obtained.
  • Gathering social information related to residents and patients.
  • Maintaining awareness of current community resources available to residents, patients, and family members.
  • Coordinating all discharges within the community.

Requirements
Minimum Education, Experience and Skills :
Education: Associate's degree
Valid driver's license issued by the State of South Carolina
Experience: One year of skilled nursing experience
One year of Medicare admission experience
Preferred Education, Experience and Skills :
Education: Bachelor's degree in the social services or related field
Experience: Two years of skilled nursing & Medicare admission experience
  • Excellent communication and interpersonal skills.
  • Ability to empathize and connect with residents, patients, and family members.
  • Strong organizational and problem-solving abilities.
  • Proficiency in Microsoft Office Suite and other relevant software.
