The Social Worker's role is to respond to each resident's social needs from the time of admission and throughout the resident's stay, collaborating with other staff and professionals both in and outside the facility in developing and implementing coordinated individualized plans of care and supportive services.Social Workers must embrace the person-centered care approach of Presbyterian SeniorCare Network based upon person-directed values and practices where the voices of elders and those working and living with them are considered and respected. What we are looking for:
We need a Social Worker who will help our residents by supporting their daily living needs and develop trusting, caring relationships. Our Social Workers are required to:
  • Have a Bachelor's degree from an accredited college or university or four years of relevant experience in a long-term care community, or an equivalent combination of education and experience;
  • Be able to effectively interact with residents, resident families, and team members;
  • Have working knowledge of Internet Software, Spreadsheet Software, and Word Processing Software.
A degree in Social Work, Gerontology or other related field is preferred.
