Salary: 24.00 - 40.00 Hourly
The Social Services Specialist will travel to our Skilled nursing Centers OR, WA, ID, NV, and AK as needed (Most of the travel will be in the Portland Metro area Prefer candidates who reside in OR or WA State This position can be Full time/Part Time/On-Call Schedule: Monday - Friday POSITION SUMMARY The Social Services Specialist provides interim support to skilled nursing centers as a Social Services Director and acts as a general social services resource to assigned skilled nursing centers as necessary. When acting as an interim Social Services Director, the Social Services Specialist directs and oversees the social services program in a skilled nursing center to assure that the medically related emotional and social needs of the resident are met and maintained in accordance with all applicable laws, regulations, and Prestige Care standards. ESSENTIAL DUTIES AND RESPONSIBILITIES
Meets with all new admissions to document the resident's psychosocial history and identify the current psychosocial needs
Ensures comprehensive psychosocial care plans are in place for each resident and are completed within the required timeframe and updated quarterly and as needed
Collaborates with the leadership team, the resident and/or resident's representatives, and physician to determine discharge plans. Communicates regularly with all parties regarding the plan and encourages resident and/or resident's representative participation in all steps of the planning and discharge process
Coordinates home health, hospice, and support services, including making referrals and obtaining services from outside agencies as needed
Arranges ancillary services including but not limited to dental, ophthalmology, and audiology services. May be required to coordinate appointments for such services and arrange transportation
Completes and issues Notice of Medicare Non-Coverage (NOMNC) and Skilled Nursing Facility Advance Beneficiary Notice (SNFABN) as applicable
Completes assigned sections on the MDS and participates in the coordination of Medicaid services
Documents care plan and services provided in the resident's record in accordance with state and federal regulatory requirements and Prestige policies. Ensures that all entries are accurate, timely, and descriptive of the services provided
Acts as a resident advocate by ensuring that services are provided in the resident's best interest and educates on residents' rights
Acts as the grievance officer and coordinates the center's grievance process
Assists residents in accessing mental health services and other community resources
Arranges and obtains needed durable medical equipment, clothing, and personal items for residents
Conducts and assists in coordinating resident and family support groups including the development of programs for special needs residents
Manages behavioral health services including tracking, meetings, care planning, care and services. Coordinates and updates Behaviors Monitors for residents as required
Partners with the clinical team in training staff on resident's behaviors and appropriate behavioral interventions
Participates in leadership team by actively contributing to decision making, grievance follow up, building and company-wide initiatives and attending leadership team meetings such as Stand-up, Managing Acute Care Conditions (MACC), Quality Assurance Performance Improvement (QAPI) and other meetings as required
Acts as a subject matter expert on social services. Partners with the Senior Social Services Consultant to facilitate process improvement and drives comprehensive social services results
Partners with assigned center to implement or improve the social services program to ensure compliance with federal, state, and local requirements and company policies
Assists with onboarding, orientation, and mentoring of new social services team members and provides ongoing training and support as needed
Collaborates on special projects and strategic social services initiatives as requested
Assists with center's compliance and social services audits and plans of correction as assigned
Performs other duties as assigned
QUALIFICATIONS AND EXPERIENCE Must hold appropriate state licensure if required under state regulations. Minimum of three years' experience working in skilled nursing or similar health care setting or ability to demonstrate equivalent knowledge and skills. Knowledgeable of the rules, regulations and guidelines that govern nursing care facilities. EDUCATION Bachelor's degree in social work, human services or related field required.
